For UC Davis staff who do not have Banner Grad Track$ access.

• Use this form to initiate the payment of fellowships (stipend, fee, and/or nonresident supplemental tuition fellowships) to be paid from departmental funds to academic graduate students (

• The account from which fellowships are funded must have Higher Ed Function Code of “FINA”.

Refer to FIS 11 (Account Lookup) to confirm the account has a graduate student FINA higher education code.
