NUR 220 Nursing Care of the Adult Client I

Guidelines for Completing the Patient Plan of Care

The purpose of the graded Patient Plan of Care learning activity is to further enhance/promote the students' use of the nursing process as the foundation of professional practice.

Objectives: By completing this learning activity the student will:

1.Collect data from a variety of sources using the Client Assessment Abbreviated Format and Nursing History.

2. Describe Health alteration(s) (both textbook and clinical picture) including psychosocial and physiological alterations with signs and symptoms.

3. Interpret laboratory data including appropriate nursing interventions.

4. Formulate three actual or potential nursing diagnoses validated by subjective and/or objective assessment data in priority order. Prioritize list of all actual and potential nursing diagnoses supported by date.

5. Identify client goals with outcome criteria for each of the three diagnoses.

6. Identify at least ten specific, realistic nursing interventions for each nursing diagnosis. Including assessment, treatment, and teaching strategies designed to meet desired goals.

7. List specific rational to support each nursing intervention. Use a minimum of two references for rationale in your care plan (not two references for each rationale)

8. Evaluate the effectiveness of the plan based on identified goals and outcomecriteria.

Evaluation Criteria:

  1. The following guides are to be used in completing the graded patient plan of care:

1.1 Client Assessment Abbreviated Formats and Nursing History from NUR 220

1.2 Nursing Process Format NUR 220

2. The paper is to be typed or legibly written

3. Turn in two copies of your patient plan of care

4. An APA reference list must be included for references used for rationale, textbook picture, etc.

  1. The paper is due at 10 am on Monday, the week after the client is selected. Five points will be deducted for each day or part of the day that the paper is late.

6. The grading sheet entitled "Guidelines for Completing the Graded Nursing Plan of Care " is included in the syllabus and should accompany your paperwork.
