Parental Permission for Cheerleading
If selected, my child,______, has my permission to be a cheerleader at Fairhope Middle School if she is selected. I understand that she must abide by the rules and regulations set forth by the coaches and the principal of Fairhope Middle School. I understand that my child and at least one parent/guardian must attend the Try-out meeting on Thursday, February 12, 2015 @ 6:00 and that ALL required forms must be turned in by February 27th or my child will not be allowed to tryout for cheerleading. I understand that my child must attend the Try-out Clinic to be considered for a position on the squad.
- I understand that qualified judges will evaluate my daughter, and we agree to abide by the decision of the judges.
- I understand the financial obligations involved if my child is selected to the squad.
- I understand the very nature of the activity; cheerleading and gymnastics carry a risk of physical injury. No matter how careful the participant and the coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. I understand the risk and will not hold Fairhope Middle School or any of its personnel responsible in the case of accident or injury at any time.

2015-2016 FMS Cheer Try-Out Clinic/Try-out Information
Cheer Clinic: Monday, March 9- Thursday, March 12- 4:00-5:30 @ FMS Gym
Over the course of 4 days, you will learn the jumps, cheer, and dance that you will perform at try-outs. You will receive the dance music via email after the 1st day of clinic so that you can practice at home.
Cheer Clinic Attire
-Any athletic style top/t-shirt (no midriffs) and any athletic shorts
-Appropriate tennis shoes with no show socks
-Hairin a neat pony tail- no ribbons
-No jewelry
Requirements/Percentages for Try-outs:
During the 4-day cheer clinic, you will learn a cheer, jumps, spirit and a dance routine. On the day of try-outs, you will perform those skills and be evaluated by 3 judges. School attendance, teacher recommendations and discipline will also be considered.

Tryout Attire

Financial Obligations-if selected for the 15-16 FMS Cheer Squad
- $200 will be due at Uniform/CampWear fitting (April 14th)
- $65 per month- Squad Tumbling- not paid through school account
- The following is an ESTIMATE of the cost that will occur with cheerleading. Fundraising opportunities will be available, but may not cover the entire balance.
