Cell Phone Agreement

Background Information

In order to provide you with a means to do your job more effectively, and to help support our increased focus on safety, you are being assigned a cell phone, which will be paid for by (School District). It is important for you to be aware of the plan details, which defines the maximum limits of the financial obligation that (School District) will pay.

The cell phone is being issued for the purposes of job related communication when you are away from your desk and a regular telephone is not available. You are asked to be sensitive to the length of time and the number of calls that you make to insure that you stay within the monthly limits of the program. Limited personal use of the phone will be allowed for unusual circumstances, family emergencies, or other issues, which are considered of high-level importance. It is expected that any personal calls and business related communication will stay within the limits of the number of minutes purchased by (School District). It will be the responsibility of the person to whom the phone is assigned to insure that the monthly limits are not exceeded.

Techniques to Help Control Cell Phone Charges

We need your cooperation and assistance to help diminish the costs related to cell phone usage. The following information should prove useful in this regard.

  1. (School District) billing cycle begins each month on the twentieth of the month. You may dial 612 for information pertaining to your individual phone to aid you in keeping track of how many minutes have been used.
  2. Whenever a regular telephone is available, and circumstances allow, it is recommended that the cell phone not be used.
  3. When calling other (School District) staff, please follow the following procedure:
  4. First choice should be to call their desk phone from your desk phone, if you have one available to you.
  5. If they are not available at their regular telephone number, call their cell phone by using either a regular desk telephone to initiate the call, or utilize the two-way radio feature on the phone.
  6. If you need to call their phone number because two-way radio communication is not practical in a given circumstance, make every attempt to initiate the call from a regular desk phone.

Other Issues

The employee will be assigned a cell phone for the number of work weeks that they are contracted to work. For example, if your work assignment (number of work days/weeks) is 12 months, you will have use of the phone for the full 12 months.If you have a work assignment that lasts nine (9) or ten (10) months you will be asked to turn your phone in at the end of your work year.

Employee Responsibilities

If an employee exceeds the monthly plan limits as defined above, the employee will be financially responsible to reimburse (School District) either by personal check or via payroll deduction for the excess costs for that month. In this situation, (School District) will furnish the employee with a copy of the monthly cell phone invoice detailing calls made, costs, etc.

The phone numbers associated with these phones are not to be distributed to other staff in the district. District staff will be directed to call the department office and that office will direct the request for service.

(School District) reserves the right to require that an employee discontinue use of the cell phone and turn it in at the request of the supervisor.

Employee Acceptance Statement

I have been inserviced on the features of the cell phone assigned to me by (School District) and agree to use the cell phone in a responsible manner and according to the guidelines stated above.
