JOB DESCRIPTION

Job Title:COMMUNITY STAFF NURSE– GRADE 4

Reports To:COMMUNITY QUALIFIED NURSE

Accountable To:OPERATIONAL LEAD - CHILD AND FAMILY SERVICES

Role Purpose

The post holder will act as a practitioner with professional accountability to effectively plan, deliver and evaluate care for a group of patients with a range of conditions and needs within a recognised clinical area.

Role Context

This role operates across a range of clinical areas within Family Nursing Home Care. Individuals at this level will be able to plan and deliver care for a range of needs including complex or high risk patients within their caseload with the guidance and support of the Community Qualified Nurse. They may be required to act across for the Community Qualified Nurse and will be able to lead with confidence.

The postholder will give guidance and support to less experienced staff and will delegate and supervise the work of Care Assistants.

The postholder will participate and encourage others to take part in the Association’s fund raising activities.

KEY ACCOUNTABILITIES

1Professional Input

1.1The post holder will demonstrate a thorough knowledge of the guidelines of the NMC and Code of Conduct and Scope of Professional Practice and apply these to maintain trust and credibility with patients, carers and colleagues.

1.2The post holder will keep fully up to date with developments within their own professional discipline and maintain an adequate awareness of developments in clinical and nursing practice and will share clinical knowledge and experience.

1.3The postholder will contribute to the development of a philosophy of care which encourages the creation of a therapeutic environment for patients.

1.4The post holder will maintain a professional portfolio which demonstrates the continuing acquisition of skills, knowledge, attitudes, understanding and achievement.

2Nursing Practice

2.1The postholder will co-ordinate the care for specified individual patients or patient groups by consistently compiling care plans which meet identified treatment goals resulting in high quality patient care. The postholder will demonstrate enhanced nursing practice by acquiring knowledge and skills in reading, interpreting and evaluating research to pursue evidence based practice.

2.2The post holder will contribute to setting standards and will maintain high quality care in the clinical area. The post holder will participate in clinical audit and will enhance nursing care by acting as a role model in terms of own care delivery.

2.3The postholder will demonstrate proficiency in performing investigations, treatment and health needs assessment for patients within their caseload in accordance with defined standards.

2.4The post holder will consistently provide high quality advice to patients, carers, colleagues with a strong emphasis on health promotion in their clinical practice.

2.5The post holder will maintain accurate and timely patient held records and produce high quality patient information.

3Team Management, Building and Working

3.1The post holder will demonstrate the ability to lead and direct a nursing team/support staff when required to do so. They will work collaboratively as a member of the multi-disciplinary team, demonstrating well established networks and developing team building skills.

3.2The post holder will understand and promote team goals and will demonstrate effective supervision of junior/support staff.

4Resource Management

4.1The postholder will develop and maintain awareness of organisational strategy and the objectives contributing to the development of the business plan for the clinical area.

4.2The postholder will demonstrate awareness of organisational fundingand resource management within the community and will act in a cost conscious manner with an awareness of the implications related both to the Association and to the patient and the factors which contribute to over and underspending.

5Training and Development

5.1The post holder will seek opportunities to develop knowledge, skills and experience and will participate constructively in Performance Review and Appraisal System, taking responsibility for life-long learning. The post holder will assist junior/support staff to identify and meet learningobjectives by using the principles of teaching and learning.

6Environmental Management

6.1The postholder will contribute to the risk management of the immediate physical environment they work in which will involve understanding and promoting all health, safety and security measures within the patient’s home environment and other working areas in line with Family Nursing Home Care’s Policies and Procedures.

7Safeguarding Children and Vulnerable Adults

The post holder will take responsibility for the safeguarding and welfare of all children, adults and/or families within their care and conduct and must be familiar with the signs and symptoms of abuse and know what to do if any such concerns are raised or are identified. He/she shall be aware of and comply with Family Nursing & Home Care safeguarding and policies and procedures.
