  1. Commercial Vehicle Inspection Forms – Vehicles used by collectors shall be inspected on an annual basis by the MN State Patrol, Commercial Vehicle Inspection Division, the State District of Transportation, Motor Transport Division or any licensed commercial vehicle inspector. Attach copies of most recent commercial vehicle inspection forms for each truck licensed with WLSSD.
  1. Commercial Recycling Opportunities – at least once per year collectors who collect mixed municipal solid waste in the WLSSD must provide specific information to commercial site customers regarding available recycling opportunities. Attach an example(s) of the document(s) distributed to commercial customers in 2016.
  1. Residential Recycling Opportunities – Collectors shall annually provide each customer with information about the proper separation of recyclable materials from the waste stream. Attach an example(s) of the document(s) distributed to residential customers in 2016.
  1. Attach a copy of an example of BOTH a residential and commercial billing statement in order to demonstrate compliance with Solid Waste Ordinance Section 5.2.3(Every Mixed Municipal Solid Waste bill or invoice sent by a Collector to a customer, either commercial or residential, shall itemize the size and number of Mixed Municipal Solid Waste Containers, the frequency or period of collection for the bill or invoice, and the amount of the Solid Waste Management Fee).
