SUBJECT: / Living Well Website
PURPOSE: / To inform case managers that might receive calls from clients seeking information about Chronic Disease Self-Management Education (CDSME) of a resource available to them and their clients. The Living Well website lists workshops and information related to CDSME, Falls Prevention and related Evidence-Based Programs (EBPs) in Washington State.
BACKGROUND: / HCS developed the site with the Department of Health (DOH) in 2011 as part of an Administration for Community Living grant to provide and sustain CDSME workshops in Washington State. HCS and DOH improved the site in late 2017.
WHAT’S NEW, CHANGED, OR CLARIFIED: / The March 2018 issue of the Wellness Education newsletter will inform HCS clients about CDSME and the availability of the Living Well website to find workshops that might benefit them. The newsletter will inform interested clients that they may contact their case managers for more information. Case managers who receive inquiries from clients can use the Living Well site to find CDSME workshops by area or by type. The public site also serves as a resource for Falls Prevention information and otherEBPs. Case managers can authorize CDSME for COPES clients under Client Support Training/Wellness Education. Clients receiving MAC or TSOA are eligible under Training and Education.
ACTION: / Case managers may access the Living Wellwebsiteto learn more about CDSME, Falls Prevention and other EBPs. They may authorize services under COPES, MAC and TSOA.
