Dear Mr. Smith,
I am writing to you to introduce myself and obtain some information about your firm for my patients.
My name is Dr. John Sample and I have been practicing at 4321 State Street in Yourtown for the past 7 years. I am a graduate of Palmer College of Chiropractic, the largest chiropractic college in the Midwest. I have a general family practice and have post-graduate training in ______. In addition, I have ______.
On occasion, I have new or existing patients present to my office with injuries sustained during auto accidents, slip and fall type accidents, or work related injuries. We are well equipped to handle these type of trauma cases, but I am not well equipped in determining liability or in answering my patients’ legal questions regarding their rights or obligations.
I would like to establish a list of three or four attorneys that I can recommend my patients contact to handle any possible legal situations that may be involved in their case. I would like to meet with you to get a feeling for how you practice, what your expectations are regarding the doctor-patient-attorney relationship, and what my patients can expect should they contact your office.
