For Boarding and Day Care Clients

Just Like Home Doggie Hotel and Grooming offers an optional daily pet insurance. Our facility is an open social environment versus a kennel environment for dogs. For this reason, there is increased risk involved, including but not limited to dog fights, bites, scratches, increased vulnerability to kennel cough, parasites, upset stomach, etc. Our trained staff is here to monitor the dogs and accommodate our pet guests to ensure their safety, however, injuries and medical illness can still occur under the best circumstances. We highly suggest the insurance so we can continue to commit to the quality of service that you and your pet deserve.

$1.00/day per dog covers up to $300.00 in vet expenses per visit.

$2.00/day per dog covers up to $600.00 in vet expenses per visit.

If you choose one of the insurance options, the total insurance amount will be added to your dog’s overnight stay or day care. Payment is due at the time of check out. If you do not choose insurance, you will need to provide us debit/credit card information in the event of an emergency so that we can provide treatment for your dog.

The insurance option is non-refundable whether used or not used ______(initials).

Owner has up to 48 hours after pick up to report (call in) any suspected illness after leaving Just Like Home. If pet is taken to owner’s veterinarian, a detailed invoice will be needed in order to receive reimbursement for covered expenses. After 48 hours, there will be no reimbursement.

WHAT THE INSURANCE COVERS: Diagnostic and/or treatment expenses by a licensed veterinarian directly related to an illness or injury that arises as a result of the pet’s visit to include and limited to: Dog bites, open wounds, eye infections, broken bones, sprains, torn nails/pads, UTI’s, parasites, kennel cough (if 6 month bordetella is current 12 month bordetella is not covered for kennel cough), bee stings. _____ (initials)

WHAT THE INSURANCE DOES NOT COVER: Pre-existing conditions, age-related illnesses, injuries caused by same family pets, heart failure, vaccinations, spay, neutering, minor scratches, diarrhea from stress colitis, ear infections, allergies. ______(initials)
