Product Description: Please provide an attachment with a list of merchandise with a clear description of items. Pictures are preferred in order to reduce duplication.

NOTE: NO WATER SALES-PHOENIX BLUES SOCIETY IS THE WATER VENDOR

Merchandise vendors may bring canopies no larger than 10x10 along with tables and chairs. No staking is allowed at the park. Please let us know if you are bringing a trailer: the park has specific regulation regarding trailers.

You may bring your own ice chest for drinks and snacks for yourself. Drinks must be non-alcoholic and in plastic bottles or cans. No glass allowed. Food and drinks cannot be shared with event patrons.

$25 application fee (non-refundable)

$75 merchandise vendor fee

LICENSING/PERMITS

Merchandise vendors must provide copies of:

·  State of Arizona and City of Phoenix privilege and use tax licenses

SIGNAGE

Merchandise vendors may display signage at their assigned areas with the name of the vendor, pricing, etc. Signage may not be staked into the ground as outlined in park regulations.

SANITATION

Trash boxes will be on site. Vendor is responsible for ensuring their area is clean and left in a pristine condition. Failure to maintain a clean area will result in you not being invited back.

LIABILITY

Phoenix Blues Society will not carry liability or accident insurance to cover vendors. Involvement at the 2018 Blues Blast is at your own risk.

LOAD IN/LOAD OUT

Vendors may begin at M. T. Hance Park on Saturday, March 10, 2018 by 7:00AM to load in. You must be completely set up by 9:00AM. Late load-ins may result in exclusion from event and forfeiture of vendor fee unless you carry in everything by hand. Vendors may not load out at the end of the event until the event participants have left the park. Event volunteers will be on site to identify where to place your table, etc.

CANCELLATION POLICY: Event will take place rain or shine. No refunds will be provided if you cancel after March 1, 2018. Cancellations must be received by February 28, 2018 (either by mail or email). There will be a $25 processing fee for cancellations. Cancellation requests made after February 28, 2018 will result in forfeiture of application and vendor fees.

CONFIRMATION POLICY: Vendors will be notified of their acceptance for participation once they are selected and the required payment has cleared.

HANCE PARK LOAD IN / LOAD OUT PROCEDURES

Dear Festival Participant:

To assist you with quick and easy load in and load out procedures for Hance Park please observe the following guidelines.

·  No staking of any kind in the ground is permitted – due to irrigation systems, fiber optic cables and because we are on top of the I-10 tunnel freeway and the land belongs to ADOT. We all must follow ADOT policies regarding weight limitations, etc... No vehicle exceeding 10 tons (20,000 lbs.) will be permitted onto the park or the 3rd Street parking lot.

·  Minimal driving on the park is permitted and must be supervised and pre-approved by City of Phoenix staff.

·  You may quickly unload your vehicle and proceed to park in your assigned area given to you by event producers. If no area has been assigned, only park in legal parking areas. ALL LIBRARY LOTS ARE OFF LIMITS. Please do not park in areas designated NO PARKING. You could be ticketed and/or towed.

·  Please do not set up your booth while unloading your vehicle. If pre-approved by City of Phoenix staff to drive onto the park, please unload your vehicle first, then return to designated area to set-up your booth or designated spot.

·  Please do not drive over, near or on areas that are marked with ORANGE CONES. These areas are marked for a reason, usually the main valves for the irrigation systems.

·  Vehicles will not be allowed onto the park until at least ½ hour after event closing time or until City of Phoenix staff determine it is safe to do so.
