Nomination Form and Procedures

The Bowling Green Youth Hockey Association is pleased to announce the initiation of the Mark R. Nelson Memorial Coaching Award. Mark Nelson (1954-2012) was a long-time, inspirational coach of youth hockey players in the Bowling Green area. To honor Mark’s life and legacy, the Mark R. Nelson Memorial Coaching Award will be presented annually by the Bowling Green Youth Hockey Association to a youth hockey coach who fully exemplifies Mark’s outstanding characteristics as coach, role model, and friend to youth. NOMINATION INSTRUCTIONS

1. All nominations must be submitted using this form, which is available online at .

2. Nominations may be made by anyone with a significant interest in Bowling Green youth hockey. No self-nominations are permitted nor are nominations by members of the Award Committee.

3. Nominations must be accompanied by a minimum of two letters of support documenting the indicated nominee’s qualities and characteristics that embody and reflect Mark Nelson’s characteristics and contributions as coach, role model, and friend to youth. Each letter should not exceed one page typed.

4. Nominations are due this year only by January 15th. Nominations may be delivered in person to the BGSU Ice Arena, sent to Bowling Green Youth Hockey Association, P. O. Box 123, Bowling Green, Ohio 43402, or emailed to BGYHA President Roger Schick

5. Nominations will be reviewed and selected by an Award Committee comprised of two representatives from the BGYHA, and one representative named by the family of Mark Nelson.

6. The award shall be presented at the annual Youth Hockey Day event, to be held this year on Saturday, January 27th, 2018. Attendance of the award recipient is expected.
