Admissions Process

We appreciate your interest in Village Learning & Achievement Center. We are a community-based program for people with developmental disabilities. We support the physical, emotional and social well-being of the Villagers through a wide range of programs that are tailored to each individual’s ability and potential. The Day Habilitation Program at Village at Plum Valley provides services for adults (18 years and older) with developmental disabilities. In addition, we provide an after-school program for adolescents (14 – 22 years). Village at Stoney Glen is an assisted living facility for individuals with disabilities (18 years and older). Temporary out-of-home respite services are also offered at Village at Stoney Glen (18 years and older). Our admissions process has several steps which are outlined below.

1. Initial Inquiry

After reviewing our web site, please contact us by phone or email with a brief description of your needs and the programs and services you may require.

2. Tour & Application

After discussing your needs with us, if you feel Village Leaning & Achievement Center can provide the programs and services the applicant requires, please contact us to make arrangements to schedule a tour of Village at Plum Valley (Day Habilitation Program) and/or Village at Stoney Glen (Residential Assisted Living Facility). During your tour you will learn about our history and our plans for the future. At the conclusion of the tour, you may have the opportunity to meet with our Director of Operations or another staff person so you may ask questions pertaining to your specific situation.

If you determine that Village Learning & Achievement Center has the programs and services to suit your needs, please complete an application form (available on our website). You may complete this form in advance of your tour and turn it in to us when you arrive. Our Admissions Committee will conduct a complete review of the information you provide. Depending upon current placement availability, you will be notified as soon as possible if the applicant is being placed on a waiting list or if a place in the appropriate program(s) is currently open. (Please note it is not necessary to tour our facilities in order to be placed on our waiting list for any of our programs. You may choose to submit a completed application form to be placed on a waiting list and schedule a tour at a later date.)

3. Admissions Packet & Interview

When it is anticipated a place will be opening up in the appropriate program(s), the Admissions Committee will schedule a convenient time for the applicant, parent(s), guardian(s), and any other interested parties to meet with our staff in order to discuss the specific needs of the applicant and family. Several weeks prior to this meeting, you will be asked to complete an Admissions Packet and submit it to us with a one-time $50.00 fee.

4. Admissions Committee Recommendation

The Admissions Committee will notify you of their recommendation. It is possible that the Admissions Committee may determine that further evaluation may be needed prior to making a decision regarding admission of a candidate. Candidates may be recommended for admission on a probationary basis. Although it is rare, a candidate may not be recommended for acceptance as a client.

5. Enrollment

If the candidate is accepted, our staff will schedule a meeting with the candidate’s family in order to complete the final enrollment paperwork. The staff and the family will develop an individualized plan for the candidate. Long-term goals and plans will be delineated at this time.
