Authorised Signatory Form
When applying to set up Security Administrators [SA] on IMS the SIGPRO form needs to be signed by a person within the applying organisation.
This person is termed as the “Authorised Signatory”, and should be someone who has suitable authority to agree the application. Usually this is the Chief Executive, Finance Director, or Chair of Governing Body.
An organisation can nominate up to 4 authorised signatories. A separate form should be completed for each.
Where the Authorised Signatory is also due to be the SA the SIGPRO form should be signed by an alternative signatory i.e. the same person cannot authorise themselves to be set up as an SA.
Occasionally an organisation needs to change their authorised signatory due to changes within their organisation. To do this simply complete a new form and send it to the address provided.
The HCA no longer requires a hard-copy of the form. However we do need the signature of the Authorising person to be an original, signed by hand and NOT an e-signature or photocopy.
The process to set up a Security Administrator takes up to 48 hours from when we receive the form to when the email confirmation is sent out. The SA will then need to set up the organisation users. As far as possible please allow time for getting set up on IMS as part of your programme planning.
When a request to update a Security Administrator is received, the signature will be checked against the authorised signatory held for the organisation. If these do not match you will be asked to send and/or nominate a new authorised signatory and resend the SIGPRO form with the new signature.

Checklist on completion of form
1)Complete and print the form on the provider’s headed paper. This should match the organisation applying for access.
2)The name and address of the organisation has been completed.
3)The authorised signatories’ name and telephone number is noted on the form.
4)The authorised signatory is of a suitable level e.g. Chief Executive; Finance Director; Chair of Governing Body. This is a definition of the Terms and Conditions.
5)You may have up to 4 authorised signatories for the organisation, please complete a new form for each authorised signatory.
6)Scan the completed form and send to the HCA service desk .
