General IRB Submission Checklist

The following form is provided as a guide during your IRB submission process, it is NOT REQUIRED as part of your submission for IRB review. However, we recommend that you use this form to ensure that you have completed all necessary submission steps, which will minimize the chances that you will need revisions to your proposal after initial submission.

A. Checklist for IRB Proposal Submission
Check the box next to each item below, to indicate that you have addressed/completed each item.
1. Will the populations/participants in you research be defined as of the following vulnerable groups: Minors (under 18), pregnant women, prisoners, mentally retarded, or mentally disabled? “Be defined as” means, for instance, they are recruited becausethey are pregnant, not “some participants might be pregnant.” If so, the rationale for using these participants and a description of how you will protect the rights of these participants must be clearly outlined in your Nature of the Research Activity section of the Request for Review of Research form.
2. Will persons participating or recruited in this investigation be subjected to physical discomfort, pain, aversive stimuli, or the threat of any of these? If so, the procedures and related debriefing practices must be explained in detail in your Request for Review of Research form.
3. Will participants be deceived (actively misled) in any manner? If so, the procedures and related debriefing practices must be explained in detail in your Request for Review of Research form.
4. Have all investigator(s) named in this proposal (including faculty advisers) completed the appropriate CITI ethics training courses? Trainings are available at: If your proposal is submitted for full review (i.e. is not exempt) you will most likely need to complete the Social/Behavioral research course from CITI.
5. Have you included in your submission to the IRB, a completedManhattanville College Institutional Review Board Informed Consent Form Checklist?
6. Have you included in your submission a copy of your Informed Consent Form(s)?
7. Have you included in your submission copies of all questionnaires, surveys, instruments, or tests to be administered to the participants?
8. Have you included in your submission copies of all recruitment emails, posters, or oral scripts?
9. If you are a student, has your research advisor/professor electronically signed your IRB proposal form?
10. For faculty members, have you submitted all documents, with your last name in the file name of each attachment, to ? .
