IELD TRIP MEDS

Please read carefully

1.  If your student will need daily medications and/or as needed medications while on the Washington, D.C. trip, please review the following information. Medication administration procedures for the trip are in compliance with the Ohio Department of Health medication administration regulations and the Chagrin Falls Exempted Village Schools medication administration policy.

2.  Students who already have medications @ school (e.g. EpiPens, Benadryl, inhalers, advil) do not have to resubmit orders or medications; their school medications will be taken on the trip.

3.  In order for your child to receive any medication (except as noted in item #2 above) while on the field trip please conform to the following:

ü  A Request to Administer Medication form(s) must be completed and signed by BOTH the parent/guardian AND DOCTOR for each medication to be given during the trip. This form is used for both medications taken on a daily basis and ‘as needed’ (e.g. Tylenol, Advil, etc.).

o  Request to Administer Medication forms are included in this mailing.

o  Additional forms are available in the MS office and on line under the Heath Services tab on the Middle School webpage.

ü  When Tylenol and/or Advil (or generic equivalents) are ordered for a student, school stock (pills) will be administered (with a Request to Administer Medication form completed and signed by BOTH the parent/guardian AND DOCTOR). Any other over-the-counter medication must be supplied by the parent/guardian.

ü  All medication(s) must be brought to school by a parent/guardian, in its original container. Over-the-counter medication bottles/packages must be new with an unbroken seal. Prescription medications need to be in a bottle with a fixed pharmacy prescription label. You can ask the pharmacist for a 2nd bottle complete with the fixed pharmacy prescription label.

ü  Medication(s) and signed Request to Administer Medication form(s), must be brought to the school by the PARENT/ GUARDIAN (not by the student), NO LATER THAN FRIDAY, May 19th, 2017.

No medications will be accepted on the morning of the trip.

o  Please administer any daily morning medication(s) prior to departing for Washington.

Medication must be picked up by the parent/guardian upon return from the trip. Any medications left at school will be disposed of on Tuesday, June 6th, in compliance with Ohio Law.
