DISCLAIMER

House of New Hope has established a variety of employee benefit programs designed to assist you and your eligible dependents in meeting the financial burdens that can result from illness and disability, and to help you plan for retirement. HR 505 Employee Benefits contain a very general description of the benefits to which you may be available to you as an employee of House of New Hope. Please understand that this general explanation is not intended to, and does not, provide you with all the details of these benefits. Therefore, this policy does not change or otherwise interpret the terms of the official plan documents. Your rights can be determined only by referring to the full text of the official plan documents, which are available for your examination from the Human Resources Department. To the extent that any of the information contained in this Handbook is inconsistent with the official plan documents, the provisions of the official documents will govern in all cases.

Please note that nothing contained in the benefit plans described herein shall be held or construed to create a promise of employment or future benefits, or a binding contract between House of New Hope and its employees, retirees or their dependents, for benefits or for any other purpose. All employees shall remain subject to discharge or discipline to the same extent as if these plans had not been put into effect.

As in the past, House of New Hope reserves the right, in its sole and absolute discretion, to amend, modify or terminate, in whole or in part, any or all of the provisions of the benefit plans described herein, including any health benefits that may be extended to retirees and their dependents. Further, House of New Hope reserves the exclusive right, power and authority, in its sole and absolute discretion, toadminister, apply and interpret the benefit plans described herein, and to decide all matters arising in connection with the operation or administration of such plans.

For more complete information regarding any of our benefit programs, please refer to the Summary Plan Descriptions, which were provided to you separately or contact the Human Resources Department. If you lost or misplaced those descriptions, please contact the Human Resources Department for another copy.

Summary:

Eligible employees at HONH are provided a wide range of benefits. A number of the programs (such as Social Security, workers' compensation, state disability, and unemployment insurance) cover all employees in the manner prescribed by law.

As a benefit to our employees,

HONH contributes a monthlystipend to each full-time employee for the purchase of eligible benefits.

Full-time PCA and RA who opted for the benefits wage option will receive a per month stipend for the purchase of medical and/or dental insurance only.

Guidelines:

Benefits eligibility is dependent upon employee classification. Human Resources can identify the programs for which you are eligible..

All full-time employees (including PCA and RA) are eligible for an employer-paid group term life insurance policy. Associated value-added benefits may include:

The following benefit programs may be available for election by eligible employees through the Agency’s Section 125 Cafeteria Pre-Tax Plan and subject to change without notice. In many cases, the premiums for these benefits are pre-tax:

Benefits Stipend:

HONH provides all full-time employees with a monthly contribution toward the purchase of allowable desired benefits. Depending upon their election of benefits, employees may also contribute to the cost of their benefits. Any portion of the monthly agency contribution not used by an employee is returned to the agency and cannot be converted to cash or applied elsewhere.

Regular full-time PCA and RA who selected the benefits-wage option will receive a monthly contribution toward the purchase of medical insurance. Any portion of the monthly agency contribution not used by an employee is returned to the agency and cannot be converted to cash or applied elsewhere.

As an added benefit to HONH employees, many of the benefit premiums can be paid with pre-tax dollars, thus saving the employee a substantial amount of money.
