Job Description - Carers Assessment Co-ordinator

Department -Carers Services Oldham

Responsible to-Carers Services Manager

Objective

Wired is a charity that provides services to disadvantaged people including Adult and Young Carers across the North West of England and Wales.

The post holder will be responsible for carrying out Carers assessments, identifying and registering unpaid Carers in Oldham, as well as following up calls from professionals and carers. In addition, you will line manage a team of professionals who support carers.

Tasks and Responsibilities

Line Management

a)Provide direction and supervision to the Carers Assessment teamon a regular basis.

b)Co-ordinate the work of the team and ensure that changes within Health and Social Care policy are communicated to the rest of the team.

Supporting Carers

a)Provide one to one emotional support to Carers

b)Carry out aninitial Carers Assessments or Individual Budget or Emergency Card, as required.

c)Provide Carers with information and advice which empowers them to develop other opportunities alongside their caring role.

d)Provide information to Carers that will support them to access additional support outside of the Carers Centre.

e)Refer Carers on to appropriate support services including counselling.

f)Refer more complex needs as agreed to the relevant social work team or department.

g)Provide support to Carers at our drops in, as requested.

Representation

h)To maintain up to date knowledge of relevant legislation, national and local appropriate to Carers.

i)To record information in line with good practice to ensure that monitoring information is accurate and appropriate.

j)To take part in Carers Week and Carers Rights Day as requested.

k)To ensure that all information requested by the Service Manager is provided in a timely manner.

l)To integrate feedback received from colleagues, clients and from other internal and external clients and agencies, into a continuous review process, to ensure that the service meets the needs of all users, and best practice is implemented

m)To contribute to the continual review of policies and procedures to ensure that the service is managed in the most effective manner, and provides optimum service to both clients and service commissioners.

n)Provide input to the ongoing Service Level Agreement between Wired, Oldham Council and NHS Oldham, and ensure that working practices are appropriate for the achievement of all agreed goals, targets& standards.

3.) Miscellaneous Accountabilities

a)To follow all Wired policies and procedures

b)Maintain confidentiality of sensitive information pertaining to Wired and/or CCG Oldham/ Oldham Council in line with Data Protection and Confidentiality Policies

c)To fully understand Safeguarding legislation in relation to the services delivered and the service users identified under these services

d)Collect and collate information for the purpose of regular statistical and qualitative reporting, with input from clients and staff.

e)Exhibit a professional, friendly and approachable demeanour to clients, colleagues and other professionals at all times.

f)Utilise effective time management techniques to ensure tasks are prioritised, and agreed deadlines are met.

g)Work towards goals within a Personal Development Plan to meet identified individual and business need, and attend all relevant staff training,supervisions and meetings.

h)Ensure that attitudes and work behaviour reflect the philosophy of Wired at all times (refer to Wired Code of Conduct and Equality Policies).

i)Any other duty that may be required, commensurate with job title and salary (Wired’s services are continually changing – flexibility and adaptability is essential).
