Please attach a letter explaining your reasoning for this nomination including the process that was completed to select the nominee (whether by committee vote, recommendation by a board, individual nomination, etc.). Please focus on ways that the nominee has given back to the community. Include how the nominee meets the criteria on the attached page, any known awards presented to the nominee, significant dates, milestones, or contributions applicable to the nominee, service projects or other significant acts done by or in conjunction with the nominee, photos, videos, articles, or other features that showcase the nominee, and any additional information that might help the selection committee determine an award winner. Project nominations must also include a letter from a sponsoring organization describing in detail what they consider the most significant aspect of the project. Eligibility is based on the attached criteria for each award. The deadline for submissions is 5:00 p.m. Friday, October 14, by mail, fax, or email or in person to the Chamber office. No submissions will be accepted after this deadline. Awards will be given at the Annual Banquet, Monday, November 7, beginning at 6:30 p.m. Tickets are available through the Chamber.

2016 Annual Banquet Award Nomination Criteria

Citizen of the Year:

·  must be a resident of Conway County

·  must be actively involved in the community

·  must demonstrate good character in responsibility and citizenship

·  must demonstrate community service in more than one area

Educator of the Year:

·  must be an educator in a Conway County school or university

·  must inspire students of all backgrounds and learning abilities

·  must play an active role in the community as well as the school

·  should have the respect and admiration of students, parents, and colleagues

Youth Citizens of the Year: (from each Conway County school)

·  must be a 2016-2017 senior at each of the Conway County schools

·  must demonstrate good character in leadership, responsibility, fairness, and citizenship

·  must demonstrate service in scholastic achievement

·  must be actively involved in the community

Business Site of the Year:

·  must be an active member of the Morrilton Area Chamber of Commerce

·  must demonstrate community support

·  must maintain a customer friendly appearance

·  other areas considered: attractiveness, cleanliness, maintained facility (inside and out)

Community Project of the Year:

·  must improve the quality of life in Conway County

·  must exhibit community enhancement, cooperation, and participation

·  project must have exhibited some activity from Nov. 2015-present

Education Project of the Year:

·  must be completed in a Conway County school

·  must be unique, interesting, or innovative

·  can be a collaborative effort with a non-educational group/institution

·  can be an established or new project
