Modified Attendance Agreement

This form will act as an agreement between the student and the instructor as to how disability-related absences will be handled and to set guidelines for the student to follow and achieve completion of the course with the agreed upon modifications.

Students should understand that:

  • The Modified Attendance Agreement only applies to disability-related absences. Non-disability related absences are not excused and are subject to attendance policies as outlined in the course syllabus.
  • Accommodations, including the Modified Attendance Agreement, are not retroactive. The details of this accommodation begin on the date that the instructor and student sign the agreement.
  • The student is responsible for missed course content due to disability-related absences and must meet the same course requirements as outlined in the course syllabus.
  • Unless otherwise specified in this agreement, all assignments and exams must be completed by the last day of class for the term.
  • If absences exceed the agreed upon limit, penalties will occur as outlined in the course syllabus.

How and when should the student notify the instructor of disability-related absences?

The student must contact the instructor within 24 hours of the initial date of the absence.

Classroom Attendance

What is the maximum number of absences a student can miss related to their disability?

% of the course grade is connected to attendance, in class assignments, and class participation. If the student is absent for disability related reasons, s/he may meet with the professor to make up those assignments.

The student will be allowed to miss up to …….. additional classes for disability-related reasons beyond what is stipulated in the course syllabus. After that, penalty as describe in the course syllabus, will occur.
These missed classes will not impact the attendance grade, but could impact the overall learning. It is up to the student to make up any missed course material.

Exams & Quizzes

What is the procedure for making up a missed quiz or missed exam due to disability-related absences?

The student will be allowed to make up exams and or quizzes within 24 hours of the original quiz or exam date. This exam can be taken with the TA or in the DAS Testing Room.
Note: This does not apply to a missed Final Exam. Missing a Final Exam may warrant an incomplete (I) grade for the course.
Courses in which test or quiz grades may be dropped as part of the final grade calculation: this should not be applied when a student misses a quiz or exam for disability-related reasons.

Assignment Due Dates

What are the procedures for turning in assignments missed due to disability-related absences?

The student may submit assignments within 24 hours of the original deadline
or
Assignments due dates missed for disability related reasons may be turned in at the next class period or can be submitted on Moodle.
or
The student has one week to compete the assignments if s/he missed the due date because of disability related absence.
