Hi everyone. Today I will walk you through the data collection tools located on the project website. / Slide 1

Before we get started, I would like to introduce myself. My name is Sarah Islam, and I am a Senior Research Analyst at NORC. I manage many elements of the website and will participate in analyzing the data you share with us. / Slide 2

  • Log into the website and access data collection tools
  • Understand the common features of the tools
  • And preview the following tools for Acute Care settings: 1) structural assessment, 2) hospital survey on patient safety culture, 3) team antibiotic review form, and 4) clinical measures

How To Access Data Collection Tools
SAY:
To access the data collection tools, first log in to the website with your unique credentials. If you have not yet received this information, email with your first name, last name, and email address. / Slide 4

After You Login, Click on “Data Collection Tool”
SAY:
After you log in, click on “Data Collection Tool” on the top banner. We will notify you when data collection begins but until then, this tab will be hidden. / Slide 5

Select “Type” and then “Collections”
SAY:
Once you click on that, the top portion of this slide will appear. I will review the drop down menus. “Type” contains the type of user.
“Collections” contain the tool.
First select “Type” from the drop down menu. You will be assigned as one of the following:
  • unit member
  • facility lead
  • coordinating entity lead
Once you select “Type”, only tools associated to that type of user will be visible in the “Collections” drop down menu. You will not be able to view tools assigned to other “Types”.
The following tools are available in the
“Collections” drop down menu for Acute Care settings:
  • structural assessment
  • hospital survey on patient safety culture
  • team antibiotic review form
  • clinical measures

After Selecting Type & Collections a List of Tools Will Appear

This page shows you an expanded view of what you will see. After you select a “Type” and “Collections”, you will see the tool in the middle of the page.
Status indicates if the tool is Open, Closed, or has been Submitted. Selecting “All” will display all the tools, including those that are open, closed, or submitted.

Common Features of the Tools
SAY:
Before showing you a preview of the tools, I want to review some common features on the website.
Each tool has dates listed next to its title to indicate how long it will be open for. You can only complete it during that timeframe.
Each tool comes with a set of instructions that you should read carefully.
There is also a progress bar to show you the percentage of the tool you have completed.
Lastly, as you fill out each page of questions, click on “Save & Continue” to go to the next page. Click on Back to go back to the previous page. / Slide 8

Common Features of the Tools
SAY:
After 25 minutes of inactivity, the website will log you out so make sure to save as you complete the tool.
If you wish to save what you have completed so far and return at a later time, click on “Save & Exit”. The system will save your progress. / Slide 9

We strongly encourage you to answer all the questions in each tool. If you do not, a warning may pop up as a reminder. / Slide 10

When you have completed the tool, you will see this window. You may review all of your answers on one page, go back to change your answers, or submit.
Once you submit, you can no longer open the tool and edit your answers. / Slide 11

Common Response Types
SAY:
Now I will review some of the different response types that you may encounter.
Multiple choice questions allow users to select responses from predefined lists. You may be asked to choose one answer or all that apply. / Slide 12

You may be asked to record numbers and percentages. The tool will notify you if non-whole numbers are accepted. / Slide 13

The first one I will preview is the Structural Assessment, which collects basic information about your institution or units as well as the experience of your team-based safety program and activities related to antibiotic use. / Slide 16

Structural Assessment: Instructions

When you open any tool, the first page will be a set of instructions. Generally the instructions will introduce the tool and briefly discuss the type of information collected. It will also indicate when the tool is open and review some basic features.

This tool is slightly different than the previous two since it allows you to complete the tool multiple times in a month. We strongly suggest you review the Completion Guide for this tool located under the Acute Care Resources page before you begin. / Slide 23

This tool is also slightly different because you will upload your data for these indicators to the website. I will not review these indicators in this presentation since they were covered in previous webinars. / Slide 26

You may upload one or more files that contain data for the indicators listed on slide 26, such as days of antibiotic therapy and rate of C.difficile infection,by clicking on “Add File”. After adding all the file names, click on each name and upload the actual file. Please use meaningful file names such as the examples listed here. / Slide 27
