RELEASE, CLEARANCE AND
AUTHORIZATION TO RECORD
I, (“Releasor”), hereby enter into this Release with the University of Washington and the UW/EDGE Program (“the University”).
Releaser hereby authorizes the University to record his or her picture and voice and any information contained in documents and other types of media provided by Releasor on photographs, films, video and audio tapes, CD-ROMs, DVDs, digital file or any other media in connection with the University of Washington’s HCDE 521, Current Issues in Human Centered Design & Engineering, and to use these recordings at the University’s discretion, and to use and license others to use these recordings in their original or edited form in any media for any and all education or commercial purposes, and to use Releasor’s name, likeness, voice, and biographic or other information in connection therewith; provided that Releasor’s prior written approval shall be required for any non-educational or commercial purpose for which such recording is used by the University, which approval shall not be unreasonably withheld.
Releasor represents and warrants that the presentation by Releasor and related presentation materials provided by Releasor will not contain any material which violates the rights of any third party or other entity including without limitation copyright, trademark, right of publicity, right of privacy, or literary, artistic, musical or dramatic right of any person or entity. Releaser represents that the presentation materials provided by Releasor are not subject to any prior agreements, or any third-party permissions, that would limit the scope of the permission Releasor is granting herein. If Releasor breaches the representations made here, and if the University should suffer damages as a result, Releasor agrees to indemnify the University for those Damages.
Releasor acknowledges that he or she will not be compensated for the University’s use of the recordings, and that the University exclusively owns all rights to the recordings, subject to Releasor’s or other third party ownership rights in any underlying presentation materials included by the Releasor and incorporated in the recordings. Releasor may use the recording for personal, educational or other non-commercial use in its unedited form without prior approval from the University.
Releasor warrants and agrees that he or she has read and understood the contents hereof, and that he or she has the right and authority to execute this Release.
