Early Registration: Register by May 1st and receive $50 off Level I/II/III or $25 off Folk Song. REGISTRATION AND PAYMENT DEADLINE: JUNE 20, 2017. Certification courses are three weeks per level, Monday through Friday. Courses are not prorated.
Mark courses you wish to take. Return this form with $50 registration fee by May 1st for early registration, made payable to KASC by check or KASC website store payment. Course fees are payable in full by June 20, 2017; cancellation will be refunded for 2018 courses only. Payment plans over 90 days available; payment schedules by arrangement.

**Optional credit fees for CSULA TBA/semester unit and are payable on the first day of class (credit cards accepted.) Do NOT send Optional Credit Fees at this time. LAUSD Salary Point Credit available to LAUSD teachers.
Refresher course: Audit any course successfully completed for $125 (not prorated).
Required Texts: List will be sent upon registration; purchase ONLINE. Approx, costs: Level I - $200, Level II -$250, Level III -$100, Choral $20. BRING your text books from previous levels. KASC scholarships available for current OAKE members, subject to Scholarship Fund balance. Request an application from Coordinator; deadline: 5/15/2017.
