JOB DESCRIPTION

TITLE: FAMILY NURSE PRACTITIONER

REPORTS TO: MEDICAL DIRECTOR

STATUS: FULL TIME, EXEMPT

POSITION SUMMARY

The Family Nurse Practitioner (FNP) will be responsible for direct patient care and for other administrative duties as designated by the Medical Director and Chief Executive Officer. The Medical Director will provide the overall direction for all patient care activities.

QUALIFICATIONS:

JOB QUALIFICATIONS AND EXPERIENCE

1.  Must possess and Maintain a Medical License to practice as a Nurse Practitioner (NP) in the State of California

2.  Possess and Maintain DEA Certification

3.  A certificate of completion from an accredited nurse practitioner program.

4.  Maintain and possess Current Basic Life Support (BLS) certification.

5.  Current Advanced Life Support Card

6.  Current DEA Certification

7.  Valid California drivers’ license, with proof of auto insurance in the amount of $5,000 property damage, and $15,000/$3,000 bodily injury.

8.  Maintain unrestricted licensure/certification to practice medicine per state guidelines.

9.  Must demonstrate knowledge of the rationale of appropriate patient care.

10.  Demonstrate the ability to maintain good inter-personal relationships with co-workers and health team members

11.  Communicates through appropriate channels

12.  Demonstrate ability to calculate drug dosages

13.  Demonstrate ability to handle emergency situations calmly and effectively

14.  Document continuing education per licensure and certification requirements and clinic protocols.

15.  Skilled in Microsoft Office, Electronic Health Records, and Billing.

RESPONSIBILITIES:

Primary Job Functions

1.  Practice medicine using sound medical judgment and adhere to all ethical considerations of the practice of medicine; know limitations and seek consult when advisable; keep abreast of current technologies and industry standards;

2.  Meet all clinical obligations of the medical practice: CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, Insurer billing/charting compliance etc.;

3.  Follow all internal policies and procedures; be accountable for completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentation;

4.  Advise administration of any issues that may impact the practice;

5.  See patients in a timely and efficient manner, stay reasonably on time for scheduled appointments in family practice throughout the day, unless exceptional circumstances occur. Patient relations, consumer satisfaction and long standing organization goals of staying on time necessitate this important consideration.

6.  The financial viability of the organization requires that all providers be held accountable for their actions with regard to proper coding and appropriate charging for all services rendered.

7.  Charting in the patient’s medical record is routinely to be completed on the same day as the patient’s visit.

8.  As a Primary Care Provider (PCP), performs the care services listed below (or be willing to learn those services, this is not a comprehensive list of services expected to be performed but are examples):

a.  Conduct complete patient history, physical examinations, diagnosis, and plan for treatment of primary, secondary, and tertiary care services as outlined in, and in compliance with the standardized protocols for Nurse Practitioners.

b.  Communicate with patient or guardian of patient regarding care plans, results of diagnostic studies, and answering patient and family questions.

c.  Organize and assist with delivery of health education for patients, including general health information and disease specific education.

d.  Coordinate patient referrals for specialized health care.

e.  Review of patient file and previous records, including allergies, problems, medications and immunization status.

f.  Order or execute various tests, analyses and diagnostic images to provide information on patient’s condition.

g.  Analyze reports and findings of tests and examination and diagnoses condition of patient.

h.  Administer or prescribe treatments.

i.  Determines and prescribes medication, dosage and schedule given the patient’s condition and allergies.

j.  Discusses any possible side effects to medication or immunization with patient.

k.  Prescribes vaccinations to immunize patient from communicable diseases.

l.  Promotes health by advising patients about diet, hygiene and methods for prevention of disease.

m.  Perform the care services listed below (or be willing to learn those services). Services may include:

i.  Minor surgery (toe nail removal, mole and cyst removal, foreign object removal, etc.),

ii.  Suturing,

iii.  Fine Needle Aspirations,

iv.  Casting/Splinting,

v.  General Orthopedic Evaluations/Management,

vi.  Review of x-rays (Radiologist reports usually available),

vii.  Basic rehabilitation for musculoskeletal injuries,

viii. General internal medicine, family medicine and pediatrics,

ix.  General Dermatology,

x.  General Eye Problems,

xi.  General EKG interpretations,

xii.  Emergency management skills,

xiii. ACLS and PALS Certification

xiv. General GYN Expertise

Administrative Relations and Accountability

1.  Demonstrate on-going support and respect for administration in all communications, either privately with staff or in a group setting.

2.  Address any administrative issues directly and in private with those concerned.

3.  Time management: routinely see patients in the time allotted, adjusting to daily patient flow issues to remain on schedule.

4.  Work productivity must demonstrate ability to see a patient load of 20 patients per day; effectively work patients into their schedules as caseload demands and respond to a variety of unforeseen daily circumstances, including emergency care of patients.

5.  Reporting: FNP’s report directly to the Medical Director and his/her designees, which include the Chief Executive Officer and the Chief Operations Officer.

6.  Must be flexible, adapt to change, and serve as a role model for staff through interactions; should at times think outside the box, providing possible resolutions/solutions to issues/concerns, rather than simply pointing out a problem area;

7.  Comply with all HIPAA rules and regulations.

Staff Development

1.  Assist in creating a constructive professional atmosphere for the staff by being a positive influence; actions/comments should enhance morale and staff relations;

2.  Serve as a resource for administration in various capacities related to a wide range of needs; meet periodically with administration at their request, either routinely or on an as needed basis;

3.  Serve as a role model for staff by setting a good example, which includes but is not limited to punctuality, demeanor, communication, customer service, work load;

4.  Learn new technologies and commit to providing leadership and training to the staff as new technologies are implemented.

5.  FNP’s working in family practice, along with all other family practice providers, are responsible for call coverage (phone coverage only).

SUPERVISORY RESPONSIBILITES

This position has no supervisory responsibilities.

- Provide customer service in accordance to clinic mission

- Demonstrate ability to be courteous and respectful when interacting with patients and family members

- Maintain patient confidentiality in accordance to policy and procedure and HIPAA requirements

- Enact proper chain of command for patient complaints
