Course Description:

Students function as PTAs under the supervision of a physical therapist, using knowledge and skills developed in the program. In addition, students conduct an in-service presentation to the clinical staff.

Pre-Requisites: Successful completion of all semester 4 PTA classes

LEARNING OBJECTIVES:

1.Meet all observed objectives of Clinical Education I and II.

2.Communicates an understanding of the plan of care developed by the physical therapist to achieve short and long term goals and intended outcomes.

3.Demonstrate competence in implementing selected components of interventions identified in the plan of care established by the physical therapist, including: developmental activities; prosthetics & orthotics adjustment and training; passive ROM; aerobic conditioning; balance & coordination training; breathing exercises & coughing techniques; conditioning & reconditioning; postural awareness training; ROM exercises; stretching exercises; strengthening exercises; application and removal of wound dressing or agents; and identification of precautions for dressing removal.

4.Identify the individual’s and caregiver’s ability to care for an assistive device.

5.Describe the safety, status, and progression of patients while engaged in gait, locomotion, balance, wheelchair management, and mobility.

6.Recognize absent or altered sensation.

7.Recognize normal and abnormal integumentary changes.

8.Recognize normal and abnormal joint motion

9.Describe resting posture in any position

10.Recognize alignment of trunk and extremities at rest and during activities

11.Recognize level of functional status in any given patient.

12.Adjust interventions within the plan of care established by the physical therapist in response to patient clinical indications and report this to the clinical instructor.

13.Recognize when an intervention should not be provided due to changes in the patient’s status and report this to the supervising clinical instructor.

14.Report any changes in the patient’s status to the supervising clinical instructor.

15.Recognize when the direction to perform an intervention is beyond that which is appropriate for a physical therapist assistant, and initiate clarification with the physical therapist.

16.Participate in educating patients and caregivers, as directed by the supervising physical therapist.

17.Provide patient related instructions to patients, family members, and caregivers to achieve patient outcomes based on the plan of care established by the physical therapist.

18.Complete thorough, accurate, logical, concise, timely, and legible documentation that follows guidelines and specific documentation formats required by state practice acts, the practice setting, and other regulatory agencies.

19.Participate in discharge planning and follow-up, as directed by the supervising physical therapist.

20.Under the direction and supervision of the physical therapist, instruct other members of the health care team using established techniques, programs, and instructional materials commensurate with the learning characteristics of the audience.

21.Educate others about the role of the physical therapist assistant.

22.Interact with other members of the health care team in patient care and non-patient care activities.

23.Provide accurate and timely information for billing and reimbursement purposes.

24.Participate in performance improvement activities.

25.Demonstrate a commitment to meeting the needs of the patients and consumers.

26.Recognize the role of the PTA in the clinical education of the PTA student.

INSTRUCTIONAL METHODS:

Onsite clinical instruction. This course emphasizes the application and integration of academic coursework in the clinical setting. Students are directly supervised by licensed Physical Therapists or Physical Therapist Assistants in clinical sites available throughout the United States.

LEARNING RESOURCE ACTIVITY AND ASSIGNMENTS:

  1. This course requires a research assignment. Students are expected to conduct an in-service presentation to the clinical staff. Assignment may include but is not limited to gathering information by using the onsite library or accessing online resources such as EBSCO or Internet searches. Submit the in-service to TurnItIn by the due date listed below and as posted on TurnItIn.
  2. This course requires completion of a Weekly Planning Form. The student will be required to identify two goals each week (work with your CI for suggestions). The Weekly Planning Form also includes a written summary for each week of clinical rotation. Examples include (but are not limited to) exercise progressions, new physical therapy techniques, hands on practice of skills learned in school, followed another discipline (i.e. OT, SLP, Respiratory, Nursing, Case Management, etc.). The weekly summary should not be limited by this list; and should include any information that you found useful during the week. When completing this assignment consider the five (5) performance dimensions: quality of care, supervision/guidance required, consistency of performance, complexity of tasks/environment, and efficiency of performance. The completed Weekly Summary Form will be uploaded to TurnItIn by the end of each week. Due dates and times will be posted on TurnItIn.
  3. Students are required to complete licensure review questions for Chapters 8 - 14 from TherapyEd by O’Sullivan & Siegelman (review questions are listed at the end of each chapter). Each chapter review assignment must be submitted to Turnitin by the deadline as posted on TurnItIn.
  4. Take Practice Exam B in TherapyEd by O’Sullivan & Siegelman. You must allow yourself three (3) hours to complete this exam. Submit exam results via TurnItIn by the due date listed below and as posted on TurnItIn.
  5. Complete the Clinical Performance Instrument (CPI) for both a midterm and a final self evaluation.
  6. Complete the Clinical Site Information Form (CSIF) ( Complete the APTA Evaluation of Clinical Experience and Clinical Instruction survey available via Quia.

NOTE: The CPI, CSIF, and APTA Survey are required to be completed prior to the end of each clinical rotation. If any of these items are not completed, a failing grade will issued which could delay graduation from the program.

METHODS OF ASSESSMENT and COURSE EVALUATION:

Clinical instructor will complete the APTA Clinical Performance Instrument a standardized valid instrument that assesses student performance during the clinical education experience. Carrington College is enrolled in the CPI PTA Online Program through the APTA. Clinical Instructors may use the online version if they have completed the CPI PTA Online Course available through the APTA. If the CI has not completed this CEU, they may use the paper version. The ACCE will provide a paper version of the CPI to CIs and students if needed.If using the paper version, the originals must be returned to Carrington College immediately upon completion of the clinical rotation. Any delay in receipt of the completed CPI could result in a delayedrelease of transcripts and a delay in graduation.

The CI and the student are required to perform a midterm evaluation using the Clinical Performance Instrument (CPI) at the half way point of the clinical rotation. At the end of each affiliation the student will again evaluate herself/himself using the CPI. The clinical faculty will also evaluate the student with the CPI. If using the paper version, the originals must be returned to Carrington College. Any delay in receipt of the completed CPI could result in a delay release of transcripts and a delay in graduation.

The student will complete the APTA Evaluation of Clinical Experience and Clinical Instruction form at the midterm and again at the end of the internship. Copies may be left at the clinical facility if they so desire.

Attendance: Students are responsible for reviewing the Attendance Policy and abiding all items in the Rules of Conduct section of the Carrington College catalog. Absences must be due to ILLNESS or EMERGENCY ONLY and must be made up at the discretion of the ACCE or clinical instructor. The facility should be notified each day of an absence by 8:00am or upon opening. If you do not notify the ACCE and clinical instructor of an absence, homework will be assigned. If you do not obtain prior approval from the ACCE and clinical instructor for a schedule change homework will be assigned.

GRADING CRITERIA:

The determination of the grade for all clinical experiences is at the sole discretion of the ACCE. Course will be graded on a Pass/Fail basis. The criteria for a pass grade will be based on:

1.Punctual attendance at all scheduled clinical visits.

2.The student is expected to be at Entry-level Performance on all criteria of the CPI VAS Ratings.

3.Review of the student feedback on the clinical experience.

4.Additional written documentation from the CI or CCCE is available.

5.Progression of performance from midterm to final.

6.Whether significant concern boxes are checked.

7.The improvement over the coursework of clinical education experience.

8.Additional verbal information from the CI and student.

9.Return of all student evaluation and clinical feedback forms on time to the ACCE

10.Completion of all assignments (e.g. in-service) in the clinic satisfactorily and on time.

11.No “Significant Concern” boxes checked on any performance criteria identified at final evaluation.

If a student is judged to be at risk and significant concerns are indicated, a failing grade will be earned in the course.

Please note that for a student to receive credit for this course, a passing grade must be achieved. Entry-level proficiency in the APTA Clinical Performance Instrument for Physical Therapist Assistants for all criteriais required.

Successful completion of the internship is a combined decision of the clinical and academic faculty based upon the clinical instructor’s and student’s written and verbal evaluation of the student’s performance in the clinic. The academic faculty reserves the right to make the final determination of the grade.

GENERAL POLICIES:

1.Working hours are those established by the facility. The student is not expected to work a longer day than any one staff Physical Therapist Assistant.

2.The facility dress code is to be guide for the student.

3.Students must have current appropriate background check documentation, CPR certification and complete immunization records. This includes, but is not limited to, MMR, HBV, DPT and TB. Students are responsible for keeping copies of their own records so that they can be presented to their clinical instructors on the first day of the internship. Copies will also be kept by the ACCE in the student file.

4.Students must be supervised by at least one licensed Physical Therapist or Physical Therapist Assistant. A supervising PT/PTA must also be located in the same premises as the student at all times.

5.If any problems or questions occur during the affiliation, consult the ACCE. DO NOT wait until a clinic visit, return to school, or assume things will improve.

6.Each facility has the authority to require extra reading or reports in order to enhance the clinical experience.

PROGRESS MEETING:

1.The ACCE or another faculty member may visit the facility near the midway point of the affiliation. Any student may not be visited during each internship.

2.The visit will consist of an informal discussion with the clinical instructor, the student and possibly the Center Coordinator of Clinical Education (CCCE) to review the clinical site, the student’s performance and the clinical experience.
