Agree to the following:
1. Acknowledge, agree and represent that I understand the nature of the activity, and that I am qualified, in good health, and in proper physical condition to participate in such activity. I understand that all health and physical conditions must be described in the Health Evaluation Form. I agree to promptly express any health or safety concern to the program staff or other appropriate individuals. LUC may (but is not obligated to) take any actions it considers to be warranted under the circumstance regarding my health and safety. I agree to pay all expenses relating thereto and release the, University from any liability for any actions.
2. Fully understand that participating in such an activity may sometimes be dangerous, and I agree to assume all risks associated with my participation in events sponsored by Loyola University Chicago. I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the activity.
3. Understand that LUC reserves the right to make cancellations, substitutions, or changes in case of emergency or changed conditions or in the interest of the Program.Institutional Arrangements - I understand that LUC does not represent or act as an agent for, and cannot control the acts or omissions of, any transportation carrier, hotel, tour organizer, or other provider of goods and services involved in the program.
4. Hereby release, discharge, and covenant not to sue the administrators, directors, agents, officers, members, volunteers, and employees, other participants, organizers, and/ or Loyola University Chicago, from all liability, claims, demands, losses, or damages on my account caused or alleged to because in whole or in part by the negligence of the University or otherwise, and I further agree that if, despite this release and waiver of liability, assumption of risk, and indemnity agreement I, or anyone on my behalf, makes a claim against any of the university staff, students or participants, I will indemnify, save and hold harmless each of the releases from any litigation expenses, attorney fees, loss, liability, damage, or cost which any may incur as the result of such claim.
5. Fully understand that I am required to abide to the LUC Community Standards and any applicable rules and regulations. LUC through its official representatives has the authority to establish rules of conduct necessary for the operation of the program during the entire period of the program, including free time. I acknowledge that I will be subject to any sanctions enforced by LUC if I am not in compliance with these rules and regulations. I fully accept and assume full responsibility for my personal conduct. I understand that if found in violation of any community standards I may be asked to leave the trip and pay for my own transportation back to Chicago.
“Article 3 Section 304. University Travel, clearly states, Student are responsible for upholding the Loyola Community Standards regardless of location. This is especially important when traveling with a program sponsored by the University or as a representativeof the University. This includes but is not limited to alternative break trips, travel for athletics,service projects, and travel associated with recognized student organizations. All reports of student misconduct are received by the OSCCR will be investigated and may result in disciplinary action. Students who are on University Probation may not travel onbehalf of the University.”
I have read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, not withstanding, shall continue in full force and effect.
