Required Documents to be Attached / 1. A copy of the affection plan. (if applicable)
2. A copy of the appointment letter from the client to the applicant and contractor.
3. A copy of the professional license of the applicant.
4. A copy of the commercial license of contractor.
5. A copy of Environmental Impact Assessment (EIA) approval from DM Environment Planning & Studies Section.
6. A comprehensive design report which should include but not limited to a) detailed engineering calculations, b) Numerical modeling study of the effects of change in waves, currents, morphodynamics as well as the adjoining shorelines in the vicinity of the proposed structures, c) Soil investigation report, d) Bathymetry survey. [1 hard copy; 1 electronic copy (CD-ROM) in PDF format, bathymetry chart in AutoCAD/ Microstation format].
7. Detailed engineering drawings [2 hard copies, A3 size folded to A4, stamped by the consultant; 1 electronic copy (CD-ROM) in PDF format]
8. A geo-referenced layout of the proposed alignment of structures. [1 hard copy; 1 electronic copy (CD-ROM) in AutoCAD/ Microstation format]
9. A detailed method statement approved by the consultant. [1 hard copy, stamped by the consultant]
10. Work schedule for the major activities involved in the project. [1 hard copy, stamped by the consultant]
Notes / In cases where the contractor is not appointed, a provisional design NOC will be issued to the consultant. In such cases, a waiver for documents 4, 9 & 10 will be granted during the submission of request.
Later, on appointment of contractor, a separate construction NOC should be obtained by the consultant prior to start of construction.
