Classification / Registered Nurse Level 2 – 3 will be negotiated with successful candidate based on qualifications, skills and experience
Remuneration / Will be negotiated with successful candidate based on qualifications, skills and experience (excluding 9.5% superannuation)
Salary Packaging / Up to $15,899 per annum is available, prior to tax, as fits with RDAC’s PBI status

Direct Reports / Enrolled Nurses, Aboriginal Health Worker, Medical Drivers and Medical Reception
Probationary Period / 3 months
Working with Children Check required / Yes, upon initial appointment – then every five years
Practitioner Certificate / Yes, upon initial appointment – then renewal every year as set by AHPRA
First Aid Certificate / Yes, upon initial appointment – then every three years and CPR refresher every year
Police Check required / Yes, upon initial appointment – then every three years

Position Summary

Organisational Background
Ramahyuck District Aboriginal Corporation (RDAC) was established as an Aboriginal Community Controlled organisation in 1992 to improve the social, health and economic outcomes for Aboriginal people. RDAC is a leading provider of primary health care and related social and family support services to Aboriginal people and communities across Gippsland.
RDAC’s services are provided at the following locations: Sale (3), Morwell, Drouin, Loch Sport, Rosedale and Stratford.
RDAC’s services include; General Practice, Health assessment and education, Dental, Audiology, Chronic disease management, Drug and Alcohol counselling, Family, child and maternal health, Home and Community Care, Immunisation and vaccinations, Social and emotional wellbeing support and counselling, Allied Health, Client transport and Corporate Services.
Health Team
The Health Team’s aim to provide health care service which meets the critical health care needs of the community and to provide holistic and quality health care for Ramahyuck’s community of interest. All clinic’s are accredited against the RACGP Standards and registered with GPA+
Our health team professionals provide a comprehensive range of health care services.
Position Purpose
The Practice Nurse Coordinator is accountable for provision of nursing and management of the Drouin medical practice.
This position is responsible for coordinating and maintaining all the functional requirements of Drouin medical practice.
Specifically the Practice Nurse Coordinator will be accountable to the Regional Practice Manager:
·  Coordination and facilitation of the day to day activities of the clinic
·  Provision of and facilitation of staff development in the areas of clinical practice, patient education and the use of medical equipment
·  Oversee standards of practice of enrolled nurses, health workers, medical drivers and medical reception staff to ensure that they work within their scope of practice
·  Coordinate the purchase and maintenance of clinic equipment

The Practice Nurse Coordinator will demonstrate the following skills, knowledge and capabilities:
·  Posses appropriate Tertiary Qualification and industry experience
·  Experience working within a cross-cultural environment
·  Ability to function in a multi-disciplinary team and ability to function independently
·  Knowledge, skills and understanding of Quality Improvement processes, contemporary Human Resource Management including Occupational Health and Safety, Equal Employment Opportunity and Anti-Discrimination
·  Maintain professional standards of practice in the delivery of safe patient care
·  Develop and monitor patient chronic disease and mental health care plans
·  Practice within an evidence-based framework

Position Functions

Key areas of accountabilities
The governing principles guiding this role are to:

Leadership:
·  Provide leadership in clinical service delivery by assisting Health program staff and professionals to deliver holistic health care services in a culturally competent manner
·  Participate in a clinical role up to 50% of the time

·  The development and coordination of the multidisciplinary team to ensure the provision of optimum levels of care that meets the needs of patients in a professional and timely manner
·  Under the direction of the Regional Practice Manager, contribute to the clinical evaluation and coordination of service provision for clients, their families/carers.
·  Developing, instituting and monitoring of systems and reports that support the achievement of specific and relevant clinical and operational performance measures
·  The development of a culture within team that supports: valuing one another; exploring new ways of working together ; the promotion of excellence and continuous improvement

Service Delivery:
In consultation with relevant stakeholders:
·  Develop and monitor client chronic disease and mental health care plans
·  The application of evidenced based practice; ongoing evaluation of outcomes and continual improvement in service delivery
·  Participation in development and evaluation of service model of care and change management to support same
·  Monitor and manage patient flow in keeping with modern health care practice. Ensuring that assessment, treatment, recommendations and reports are completed and distributed in a timely manner
·  Coordinate health screening, immunisation programs and other health promotion programs
Human Resource Management:
Under the direction of the Regional Practice Manager:
·  Support the manager of Human Resources including recruitment, performance manager, leave planning.
·  Team coordination and education
·  Provision of local team leadership that ensures effective group cohesion, goal orientated coordination of effort and resolution of complex team issues.
·  Support the coordination and/or chair clinical meetings
Operational/Clinical Quality and Safety
·  Practice safely within the professional and own scope of practice boundaries. Ensure the delivery of a high standard of evidence-based nursing care
·  Establish and maintain an effective communication network with all other health professionals and support staff, consumers, families, the community and external care providers
·  Delegate responsibilities in accordance with individual levels of clinical expertise, knowledge and competence
·  Conduct comprehensive screening, assessment, planning, implementation and evaluation of care in collaboration with the patient and treating professionals to achieve goals and health outcomes
·  Maintain accurate and current clinical records ensuring documentation meets professional and legal standards
·  Ensure inventory and maintenance of equipment and supplies are current and take appropriate action in relation to repairs, replacements and deficiencies of equipment
Professional Practice
·  Practice in accordance with the Nursing and Midwifery Board of Australia 9nmBA) National Competency Standards for a Registered Nurse
·  Maintain Continuing Professional Development (CPD) as per AHPRA requirements
·  Contribute to the attainment and sharing of professional clinical expertise

Key Selection Criteria
1 / Bachelor of Nursing or recognised Australian equivalent
Current registration with APHRA / M
2 / Previous experience working in a multidisciplinary health care service or within aboriginal communities / M
3 / Demonstrated ability to maintain a high degree of organisation, coordination and communication with attention to detail and accuracy / M
4 / Experience in coordinating a health service with multidisciplinary health professionals / M
5 / Superior written and oral communication, interpersonal and client service skills with the capacity to deal confidentially and courteously with people at all levels / M
6 / Demonstrated knowledge of Health Records Act and principles of privacy and confidentiality in a health setting / M
7 / Demonstrated knowledge of Occupational Health and Safety requirements / M
8 / Proven ability to work independently with minimal supervision and collaboratively in a team environment / M
9 / Strong personal organisational skills, the ability to undertake several tasks concurrently, maintain attention to detail and meet deadlines / M
