JOB TITLE:Disability and Social Care Adviser

BASED AT:64 Altenburg Gardens, Battersea,

RESPONSIBLE TO:Service Manager

RESPONSIBLE FOR:Volunteer staff

JOB PURPOSE

  • Work in the DASCAS teamTo provide an information and advice service to residents of Wandsworth through outreach at a local GP surgery, home visits and office appointments.
  • To liaise with partnership agencies effectively to reduce duplication and increase the range of channels for accessing information and advice
  • To enable people with disabilities living in the community to identify and access benefits, social and care services.
  • To assist the Manager in training and supporting volunteer staff.

PRINCIPAL TASKS AND RESPONSIBILITIES

UNDERTAKING CASEWORK

a)Interview clients at GP surgeries, DASCAS office or their homes as appropriate, assisting them to identify their social and care needs advising them on their rights and enabling them to access services.

b)Represent clients at social security tribunals, social services case meetings as appropriate

c)Keep full and accurate case records and use and help maintain the agreed administrative systems.

d)Effectively network and collaborate with partnership organisations to deliver effective information and advice

e)Record and monitor the quality of advice and the level of service provided by DASCAS staff to ensure high standards are set and sustained.

STAFFING AND SUPERVISION

a)Provide individual volunteer staff support and supervision and share knowledge to ensure high standards of service to clients.

b)Contribute to the cohesion of the staff as a team through attending regular staff meetings and other events as appropriate.

c)Attend regular support and supervision sessions and appraisals withline manager.

d)Attend regular meetings with other members of the Transforming WandsworthAdvice Project as required.

PUBLICITY AND SOCIAL POLICY WORK

a)With the manager liaise with the Transforming Wandsworth Project Coordinator ensuring the information and experience obtained by DASCAS is used to exercise a responsible influence on the development of future Wandsworth Advice services

b)Contribute to the Wandsworth Advice Portal as required.

PROFESSIONAL DEVELOPMENT

a)Keep abreast of all new legislation relevant to DASCAS clients and of changes in legislation and practice.

b)Attend courses and conferences on new disability policies and practices, communication and management skills and use of new technology.

c)Build and strengthen relationships with other members of the Transforming Wandsworth Advice partnership.

OTHER DUTIES

a)Assist with administrative matters as required.
