I, (we), the undersigned, parent(s) do authorize First Baptist Church of Hamilton Park to obtain emergency medical care for my child in the event of an emergency. I understand that every effort will be made to locate a parent or guardian. I will not hold First Baptist Church of Hamilton Park or its Awana Ministry leaders responsible for any injury that might occur while my child participates in First Baptist Church of Hamilton Park Awana Club/24-7 Ministries.

Authorization to Participate in Physical Activities

I give my permission for my above named child to participate in Awana Ministry activities, club sports and extra-curricular activities, for the 2012-2013 school year. While expecting the Awana Ministry staff to look after the safety of my child, I hereby release the Awana Ministry and First Baptist Church of Hamilton Park Church from liability for any injury or illness that my child may sustain during club meeting and/or additional Awana sponsored activities.

General Information for 2012-2013 Year

If your child participated in Awana at First Baptist 2011-2012, you only need to complete the Medical Information and Consent Form. Please check at Awana registration on club night to verify that all information is still correct.

Why Awana and 24-7 Ministries?

Awana Club and 24-7 Ministries is a bible-centered youth organization for children ages 3 through Grade 12. The acronym Awana stands for “Approved Workman Are Not Ashamed (2 Timothy 2:15). This program seeks to help children establish a firm biblical foundation and provide practical application for their daily lives.

The Message

The heart of Awana and 24-7 Ministries is the gospel message – that Jesus Christ died for our sins and rose again.

Admission

It is the policy of FBCHP to maintain appropriate leader/clubber ratios in our AWANA program. This helps to create an atmosphere for the greatest possible attention given to each clubber. Admission to the club is on a first come first serve basis. Once the ratios of leaders to clubbers have been met, we must close admission and place you on the waiting list. Parents are encouraged to consult with the Club Director and when possible, observe in the classroom before the participant is enrolled. New and returning members may register during open enrollment at the start of our Awana year.

Enrollment Fees and Dues

The enrollment registration fees and dues for new and returning members are listed below:

· New Clubbers $45.00 (Registration $20; Club Dues $25)

· Returning Clubbers $35.00 (Registration $10; Club Dues $25)

If your child requires a new uniform, please pay cost for new clubber.
Registration covers the Cost of Books and Uniform for new clubbers only.
Club Dues covers the cost of Clubber Awards, Awards Night and Special Events.
Please complete the payment information on page 3 prior to turning in enrollment form. Completed enrollment forms can be turned in the Church Office. Checks should be made payable to First Baptist Church of Hamilton Park.

Awana and 24-7 Ministries is in session from Wednesday, September 5, 2012 until Wednesday, May 22, 2013.However, we will not meet during the week of Thanksgiving, the last three weeks in December, and Spring Break (March 13, 2012).

Our year will end with the Annual Awards Ceremony on Thursday May 23rd.

Each week we begin promptly at 6:30 and end promptly at 7:55. Clubbers should arrive 10-15 minutes early to check-in and be ready for the opening ceremony. Our schedule is subject to change, however, we will always start and end at the designated times.

Parents are required to attend Orientation on Wednesday, August 29th, 6:30 pm to 7:30 pm,

Education Wing in Room 149 C. Make-up Orientation, Wednesday, September 5th @ 6:45 pm to 7:15 pm
