Please complete the following information and return to The Gunston School with your enrollment contract. ______(print student’s name) will utilize bus transportation for the 2016-2017 school year. The fees below will be charged to your child’s student account on a monthly basis. This is a 10-month agreement. Please indicate below your bus route and if your child will be riding round trip or one way. Remit this form to Gunston’s business office.

· I agree that I will be responsible for the full year’s fee.
· I agree that this fee is non-refundable. If my child is removed from the bus for behavioral reasons, misses the stated bus pick up and departure times, is absent from school, withdraws, or is expelled from school, I agree that the money will not be refunded.
· I understand that there is a possibility that my child will not be allowed to ride the bus if my bus fee becomes past due.
· I agree that my child will abide by the GUNSTON CODE OF CONDUCT and that I will be responsible for any damages incurred on the bus.
· Please note that if fuel costs rise substantially, an extra fuel surcharge may be required.

DELAYED START SCHEDULE
School begins with the first scheduled class @ 10:00 AM, all buses pick up 1 hour and 50 minutes later than the normal bus schedule. (Delayed bus schedule is in the Gunston handbook).
