Class of 2017 MS in OT Pre-enrollment Requirements

  • All Items MUST be received by the School of Health and Rehabilitation Sciences on or beforeApril 15, 2015
  • Items 1, 3, 4, and 5 should be sent in one packet to the address below.
  • Item 5 should be completed online; submit the results with items 1, 3, and 4 in one packet to the address below.
  • You must retain copies of items 2-5for your files to be presented at all individual clinical assignments. We encourage you to keep photos of each item on your mobile phone or mobile device.
  1. Please sign, date and return the signature page of the attached School of Health and Rehabilitation Sciences Pre-professional Skill Set Standards Form. If you believe you do not meet one or more of the standards, or if you have questions, please contact us at 317-274-4702.
  1. Obtain the OT immunization packet from or the form and documentation of all immunizations as specified on the form. In addition, CC the message to Sharon Pape, MS, OTR – Academic FW Coordinator at . The Department of Occupational Therapy must retain copies of your immunization records for fieldwork placements. These immunizations are required for all students in health programs at IUPUI campus.Current immunizations must include Hepatitis Band aTB test(TB testMUST be currently valid dated 3 months or less from July 1, 2015).IUPUI Health Services provides the TB test free to current/enrolled IUPUI students with a valid university ID, no appointment necessary. Form and materials to be sent via email to the address in the packet.
  1. Submit a front & back copy of your current medical insurance card and complete the Student Health Insurance Identification Form. All graduate and professional students on the IUPUI campus are required to demonstrate proof of health insurance and maintain coverage during the period of their program enrollment. You may enroll in any health care plan; if you wish to enroll in the health care plan offered to students at IU or to learn more about the IU Student Health Plan, offered by Aetna, go to: Or you may contact a customer service representative by calling 1-812-856-4650 or via email ents must provide copies of any insurance coverage or carrier updates while enrolled in the program.
  1. Submit a printed certificate of the IU HIPAA E-Training program. Online training modules are now available in OneStart under the E Training Tab (on the left column).Launch: E Training; Select: Catalog; Select: University-wide; select: HIPAA Privacy and Security. Continue to:Students: Select: Students; Select: your Location; Select: your Program; Select: HIPAA Privacy & Security module; Enroll & Launch. After completing the training module, go to Transcripts to print certificate.
  1. A Background Certification Check is required for all students accepted into the IU Occupational Therapy program through backgroundchecks.com. Backgroundchecks.com is the only company from which the IU Occupational Therapy program will accept results. Click here to begin: the certificate ONLY. We do not need the full report.

NOTE: Some states may restrict access to practice in health professions based on criminal history. Such requirements vary from state to state regarding who can sit for the National Board Certification for Occupational Therapy Exam as well as who may practice. Students should become familiar with rules in each state before entering an OT program.
