We should be obliged if you would provide a medical report on our employee’s present health problems and future prognosis, in particular having regard to the under noted questions.

·  Does the employee’s condition prevent him/her from attending work or reduce his/her capacity to perform his/her work?

·  Is the employee’s health adversely affected by work or by particular aspects of his/her work?

·  Does his/her condition pose a risk to work colleagues or others?

·  What are the prospects and requirements for his/her rehabilitation?

·  What is the likely duration of absence?

·  Will the employee have any residual impairment and if so what are the implications for future work performance, attendance and safety?

·  Is there any need for work place adjustments?

·  If he/she is capable of doing some work, but not the work he/she is employed to do, what are the options for redeployment?

·  Does the employee’s illness, excluding the effects of treatment or medication, have a substantial adverse effect on his/her ability to carry out day to day activities?

·  If so, is it likely to last 12 months or more?

·  We would like this employee to attend [a grievance hearing/disciplinary hearing/investigatory interview/meeting to discuss performance issues/other. Is he/she fit enough to attend such a meeting and are there any adjustments that might be made by us to facilitate his/her attendance?]

Access to medical report letter to employee (required for GP report only, not OHP)

Dear [employee’s name],

Medical Report: Request for Consent to seek GP’s Medical Opinion

We wish to clarify when you may be able to return to work and seek your consent to apply to your General Practitioner (GP) for an opinion on your health in relation to your work.

Therefore, in accordance with Employers in Voluntary Housing’s Conditions of Service and under the terms of the Access to Medical Reports Act 1988, I write to confirm the Association’s proposal to make an application for a Medical Report with your consent.

Please note you have the right to:-

i)  withhold consent to the application being made

ii)  state whether you wish to have access to the report (such access is available for up to six months from the date the report is supplied)

iii)  withhold consent to the report being supplied to [Organisation’s name]

iv)  request amendments to the report via your General Practitioner

In order to authorise the Organisation to apply to your GP for a Medical Report and for this to be released, please complete and sign the letter of authorisation and return this to the Organisation in the stamped addressed envelope provided. Should you wish to have sight of the GP’s report, you must make arrangements to do so directly with your GP within 21 days of [Organisation’s name] application for this report, otherwise the subsequent rights under the Access to Medical Reports Act 1988 will be lost to you.
