REMARKS
The following notes are for items that cannot or should not be added to the template, but still should be applied to individual documents for uniformity.
Attribute Tables. Attribute tables are surrounded by a double-line border. Internal borders are horizontal lines only. The table should be left-aligned, 0.6” from the left margin, with a 0.17 space between columns. Column widths are .42, .41, .64, .48, .46, .51, .56 and 2.48. If an individual attribute table contains entries that are too wide for any of these columns, the columns can be widened. However, other columns should be correspondingly narrowed so the table has the same overall width. Attribute Table Headers should be shaded 10% and designated as Heading Rows. All columns should be centered except the last, Element Name, which should be left aligned.
Header. This should also be formatted with a Bottom Border.
Heading 1, Heading 1 Right. If you tell the Table of Contents to pick up Outline Levels 1-3, it will not pick up chapter titles unless they are in Heading 1. Follow the chapter number with a SHIFT+RETURN.
HL7 Table. These are centered, with a double outside border and full grid for internal borders. As with Attribute Tables, the HL7 Table Header should be shaded 10% and designated as a Heading Row. Columns containing text strings should be left aligned and the text should be in “sentence case” with initial letter only capitalized (though watch for proper names such as Medicare). Columns that contain only single-letter or short-code values should be center aligned.
Message Table. These have no borders. They are aligned to the left margin and indented 0.5 inch, with 0.15 space between columns. Column widths are 1.48, 3.45, and .79, with the first two columns being left-aligned and the third, Chapter, being centered. Message Table Headers are NOT to be defined as heading rows.
User Table. User-defined tables are to have no shading or borders. Text alignment is the same as in HL7 tables.
Page Setup. Top margin .75, bottom .5, header .75, footer .25. Left and right margins each 1”.
A NOTE ON HTML STYLE SHEETS
Larry devised a style sheet that uses SOME of the above styles, with suitable modifications, for applying to HTML documents. Copies of this style sheet can be found in the HTML subdirectories where the HTML copies of the Standards Guide and Implementation Guide are saved.
To imbed the style sheet into an HTML document, make sure it is copied into the same directory as that document. Then, just above the </head> tag, type the following sequence:

Styles for Heading 1-Heading 5 are in this sheet as modified -- since HTML does not support outline numbering, numbers must be manually added.
The HL7 style “Components” is incorporated into the HTML stylesheet as “definition.”
The HL7 style “Example” is incorporated into the HTML stylesheet as “defined term.” This has been modified to be in System font rather than LinePrinter, because the font the viewer sees it in depend on the fonts available on his machine; therefore Larry thinks it advisable to only use the most common fonts.
