Assumption of Risk

I am aware that ice hockey involves certain risks, dangers and hazards. The risks include, among other things potential injury from:

·  Body to body contact with other players

·  Stick on body contact from other players

·  Contact with the ice surface or the boards through falling down or being knocked down.

·  Body contact with a puck travelling at various speeds

I am aware that there is risk of loss of personal property that a participant may bring to a recreation facility.

I acknowledge and accept that personal injury, death, property damage or loss may result from participation in this program.

Release of Liability, Waiver of Claims and Indemnity Agreement

I acknowledge that my participation in ice hockey entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death or damage to myself, to property or to third parties.

I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary and I elect to participate in spite of the risks.

I hereby agree to indemnify and hold the City of West Kelowna and its officers, employees, agents and representatives, free and harmless from all liability arising out of any personal injury, death, property damage or loss sustained to me, and from all claims, demands, losses, damages, action or judgements of every kind and description which may occur to or be suffered by me through my participation in this activity, including and such claims which allege negligent acts or omissions of the City of West Kelowna.

By signing this document, I acknowledge that if I am injured or property is damaged or lost during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against the City of West Kelowna on the basis of any claim from which I have released them herein.

I have had sufficient opportunity to read this document. I have read and understood it, and I agree to be bound by its terms.

West Kelowna Adult Recreational Hockey League Rules

In order to promote the health and safety of all players participating in the West Kelowna Adult Recreational Hockey League, the rules established through the Canadian Adult Recreational Hockey Association (CARHA) Hockey Official Rule Book must be followed.

These rules include but are not limited to the following:

Rule 24 - Protective Equipment: While on the ice, including pre-game warmups, all players must wear a CSA approved helmet with the chin strap properly fastened under the chin.

A half shield visor or full face shield is highly recommended.

CARHA Insurance Facial Protection Policy - Players wearing full facial protection or half shield (visor) with an internal or external mouth guard will be allowed to submit a medical and / or dental claim for a facial injury. Member players not wearing facial protection equipment will not be insured for a facial or dental injury.

Rule 24 - Protective Equipment: Goalkeepers in Adult Recreation hockey shall be required to wear a CSA approved hockey helmet to which a CSA approved facial protector has been securely attached and not altered in any way. All goalkeepers must wear a CSA approved throat protector.

Rule 25 - Dangerous Equipment: The use of pads, protectors, or sticks likely to cause an injury to a player is prohibited.
