Dear Parent or Guardian,

The Social Studies Department will conduct a field trip on 3October 2013. We hope the following information will answer the majority of your questions concerning the Social Studies Field Trip.

Where: AmericanWestHeritageCenter (AWHC), 4025 South Hwy. 89-91, Wellsville (CacheValley), UT 84339

Why: An opportunity for students to see and experience history in a unique hands on learning environment

How: The following are the eligibility requirements:

  • Be enrolled in 7th grade Utah Studies in either Mrs. Bonham’s, Mr. Moore’s or Mr. Baca’s classes
  • Be a student who achieved a cumulative 2.0 G.P.A. or higher, and a C- or better in Utah Studies for 1st quarter mid-term, and they should have noN’s or U’s in Citizenship, in any classes.
  • Be the first fifty students todo the following:
  • (a) bring a hard copy of your Mid-Term Report Card to your respective Utah Studies teacher to prove eligibility (see above) and you will receive a stamp of approval from your respective Social Studies teacher on your Permission Slip (see attached), which will have be filled out in total, to include parent or guardian signature;
  • (b) show the stamped permission slipto Mrs. Butler and/or front office staff and pay the fee to them.

When:

  • Wednesday, 18September:field trip information letter and permission slip released to students.
  • Tuesday, 24September: last day to bring stamped permission slip and money to Mrs. Butler and/or front office staff.
  • Thursday, 03October: roll call in the auditorium and team assignments at 7:45 a.m.; load bus and leave school at 8:30 a.m., no exceptions; we will return to the schoolno later than2:15 p.m.

Notes:

  • The weather can be unpredictable. Please have your child bring a coat or hoodie. Also, wear clothing, boots or shoes that can get wet and/or muddy; no open toe shoes. Hats, gloves, and on occasion, umbrellas may also come in handy. Bring a water bottle or canteen; the drinking fountains are a long walk from the sites.
  • We recommend that your student either pack a lunch or purchase a sack lunch, which will be detailed in the Permission Slip.
  • Please don’t chase animals, and handle objects only if you are asked to by your demonstrator.
  • Please don’t run around machinery or climb on it.
  • We are still in need of volunteers to Chaperone the trip. If interested in Chaperoning, the Permission Slip will provide further information

Social Studies Department Field Trip: Permission Slip

I hereby give my permission for ______to go on the field trip to the AWHC on 3October 2013. I understand that a stamped permission slip and the fees are due to Mrs. Butler and or the front office by 24September2013.

I understand that reasonable precautions for the safety of my child are being taken. I also understand that my child must ride in district provided transportation to and from the activity unless I have made previous written arrangements with the teacher or administrator, prior to day of field trip.I further understand that teachers, administrators and other employees of Weber School District are relieved of all liability and that my student participates in this activity with the full knowledge and approval of his or her parents and/or guardian, and at his or her own risk.

IMPORTANT!!! Your student will miss important class activities while on the field trip. Each student is required to check with each of their teachers and find out what they will miss and to make up work. The field trip is to not be used as an excuse for not completing school work.

To best accommodate your student we ask that you let us know if your student has any physical limitations and/or medications that we should be aware, so we may be prepared to help them access the outdoor sites.
