IMPORTANT

Please complete the above information the same as the ‘Declaration of Conformity’; The completed forms and the required documents should be returned to TUV Rheinland by email or by fax.

TUV Rheinland remains available to provide additional information about the Product Certification to be exported to the People’s Democratic Republic of Algeria.

Please keep a record of all customer complaints made known to you regarding the product’s performance / compliance with requirements of the relevant standard and submit these records to TUV Rheinland when requested.

If you have any comments/complaints/appeals regarding the certification service, please inform in writing to our manager; further to which (if applicable) an appeal form will be provided.

This Application for TÜV Rheinland Algeria Approval Service, together with the submittal of product documentation constitutes our request to TÜV Rheinland Group to proceed with Algeria Approval Service to provide a review for clarity of the product description and our Declaration of Conformity to standards approved by Algeria Standards Organization.

1. The purpose of the TÜV Rheinland Algeria Approval Service is to provide a means for an Applicant to identify that a Regulated Product complies with Algeria requirements by means of this Declaration of Conformity, signed by the Applicant.

2. It is understood that TÜV Rheinland reserves the right to request substantiation for the Declaration of Conformity or to enforce testing of the product(s) at any time during the projects period and that such activities is at the Registrant’s expense.

3. We understand that the TÜV Rheinland Algeria Approval Service, in performing its functions as requested by us, does not assume or undertake to discharge any responsibility of ours to any other party. It is recognized that the opinions and findings of TÜV Rheinland Group, with regard to the request made by us, represent its judgment given after due consideration to the attendant limitations surrounding the circumstances and agree with TÜV Rheinland Group that TÜV Rheinland Group does not warrant or guarantee the correctness of its opinions, or that there will be universal agreement with, or acceptance of, such findings.

4. The applicant will not use, nor refer to the TÜV Rheinland Group name or logo on labels, product nameplates, brochures, advertising or sales promotion material which indicates or implies that a product is listed or approved by TÜV Rheinland Group until written authorization has been received by applicant for such use.

5. TÜV Rheinland Group will not voluntarily disclose any information obtained by it or furnished to it during the investigation of the applicant's product, without prior written authorization of the applicant.

6. This notice assumes that the applicant has read the Terms and Conditions of the TÜV Rheinland Group. If not, a copy is available upon request from your nearest TÜV Rheinland office.

7. We hereby declare that the information provided in this form is correct and the product(s) listed in the “Declaration of Conformity” of this application is/are in compliance either with (please tick ü at least one selection):

8. We understand that the product(s) listed in the “Declaration of Conformity” of this application will be subject to

¨ Pre-export inspection and

¨ Intermittent testing prior to export (if established standards are existing).

9. Any Certificate of Conformity issued in response to this application is issued strictly subject to the General Terms and Conditions of the TÜV Rheinland Group, a copy of which is available from your nearest TÜV Rheinland office.

10. TÜV Rheinland Group represents and warrants only that any Certificate of Conformity issued in response to this application is issued in good faith and in the reasonable belief that it has authority to do.

11. The application sets out the entire agreement and understanding between the parties in relation to its subject matter. The exporter acknowledges that, in submitting the application it has not relied on any oral or written representation, warranty, or other assurance (except as provided for or referred to in this application) and waives all rights and remedies which might otherwise be available to it in respect thereof, provided always, that nothing in this clause limits or excludes any liability for fraud.

12. Inspection Auditors from the local TÜV Rheinland office are to be allowed access to the Exporter’s inspection site without prior notice for the purpose of ensuring that a recently performed inspection has been correctly conducted. The Auditor is authorized by the local TÜV Rheinland office to amend or overturn the inspection outcome if the result of the audit supports this action. In their course of action, Inspection Auditors will not hamper or hinder the export process.

13. The above conditions shall be governed by the local law at the place of application and any dispute arising out of this application to which these conditions apply shall be subject to the jurisdiction of the local law.
