ACCIDENT SCENE PROCEDURES

1.  Notify authorities immediately following an accident involving another vehicle, injury or damage to property belonging to others.

2.  Do not move your vehicle until instructed to do so by authorities, unless it is unsafe to leave the vehicle in the same

location as the accident.

3.  Do Not Admit Liability. Do not make any statements that could be interpreted as an admission of liability at the accident scene. Your comments should be limited to collecting the information needed to file an accident report

4.  Obtain the following information:

If you have a cell phone with camera, and if you are able, please take pictures of the accident scene, the

damage to your company vehicle and the damage to other vehicles, or property, involved in the accident.

NOTE: If your car cannot be safely driven you should arrange for it to be towed to a repair shop of your choice. As soon as Risk Management is notified of the damage they will arrange for an appraisal. Repairs should not be started until the appraisal is completed and approved by Risk Management.

ACCIDENT NOTIFICATION

·  Physical Damage Losses – Notify Risk Management

Accidents involving loss or damage to your vehicle (collision, theft, vandalism, etc) should be reported to Risk Management.

·  Liability Losses – Notify Liberty Mutual

Immediately report all accidents involving injury, or damage to another person’s property, to Liberty Mutual’s 24/7 claims

office. Phone 1-800-362-0000. When reporting a claim, you will be asked for our policy number. The F’16 automobile

policy number is: AS2-651-0004127-045.

A written incident report must also be submitted to Risk Management within three days of the accident. You should

complete the on-line claim form (refer to the instructions above under “Physical Damage Losses”). Risk Management will

file the report with Liberty Mutual for you.

Notification to Vehicle Leasing Company

If your car is leased, promptly notify the leasing company of the accident to ensure that you are in compliance with the terms

of your lease agreement.

OTHER INFORMATION

Physical Damage Deductibles – F’16

The deductible amount will be charged to your cost center. All repair costs in excess of the applicable deductible will be

paid by Liberty Mutual or the Corporate Self Insurance Reserve (Risk Management).

Collision: $1,000 each accident

Comprehensive: $ 500 each incident (including broken windshield, theft, etc)

Personal Property- Excluded from Coverage

Personal items such as sports equipment, CD’s, cell phones, sunglasses, etc. as well as personal auto accessories such as band radios, radar detectors, GPS devices etc. are not covered under B-F’s program. You should arrange coverage for these items under your personal insurance program.

Rental Car Coverage

Coverage is provided for the cost of renting a substitute vehicle if your car is stolen or while it is being repaired after an accident. There is a 24-hour waiting period beginning with the time the car is taken to the garage for repairs or from the date of theft. Reimbursement is limited to $1,000. Any rental charges incurred during the waiting period, or rental expenditures in excess of $1,000, are the responsibility of your cost center. When renting a substitute vehicle all insurance coverage should be declined.
