Please Note: You will receive 1 pass/wristband for every fifteen (15) performers in uniform. These wristbands will allow access to the field and stands and must be worn at all times. We will also honor CBA Director and Staff passes.

Directions for completing the

CBA 2014 Music Copyright Report Form

Every band performing in a CBA sanctioned contest must submit a fully completed copyright report form WITH

all necessary supporting documents PRIOR to performing at any CBA sanctioned contest. Proof of required

PERMISSION TO ARRANGE must be submitted along with the completed form.

Note: If you are performing PUBLISHED music, and no rearranging of that

music has been done, you must still complete the Copyright Report.

1) List the name of each piece of music that your band will be performing and indicate

whether or not it is published.

2) List the name of the composer.

3) List the name of the copyright owner (if any), or if it is the Public Domain.

4) PUBLIC DOMAIN: Be sure the version you are utilizing is IN THE PUBLIC

DOMAIN. (Note if you are performing a published arrangement of music that is in

the public domain, check to see if that arrangement is protected by copyright. If so,

do not indicate Public Domain because what you are performing is a version of the

music that is protected and you should list the copyright holder).

5) List the name of the arranger (if applicable).

6) List the duration of the piece.

7) If you are performing a custom arrangement of the music, check whether or not you

have obtained PERMISSION TO ARRANGE from the copyright holder. (You must

obtain this PRIOR to performing at a CBA contest.

8) If you have obtained PERMISSION TO ARRANGE or some other form of

COPYRIGHT CLEARANCE that allows you to perform the music, check whether or

not you have included a copy of that documentation. (This must be submitted

BEFORE you will be allowed to perform at an MCBA contest).

9) If you are performing a published concert band composition or arrangement and performing it

on the marching field, you must obtain permission to perform as you are changing the composer/arrangers original intent.

10) REPEAT the above for EACH PIECE OF MUSIC that you will be performing.

11) Sign and date the form and have either your principal or superintendent sign the

form.

11) Submit fully executed form(s) to CBA or the CBA Sanctioned show host prior to your band’s

first CBA performance.
