AGREEMENT made on the ______day of ______2007 between the Women's and Children's Hospital Foundation Inc of North Adelaide in the State of South Australia (hereinafter called ‘The Foundation’) and ______(Fellow).

3FINANCIAL ASSISTANCE
The Foundation will pay the Fellow a financial grant totalling $ ______
towards the cost of fares, living allowance and other approved expenses incurred during the tenure of the Fellowship. The grant will be in the form of a lump sum payment, in advance, towards the cost of fares, living allowance and other approved expenses incurred during the tenure of the Fellowship in accordance with the budget attached to the application.
4CONDITIONS OF AWARD OF FELLOWSHIP
4.1The Award of this Fellowship implies recognition of the Fellow’s achievement in a particular field of work and that the Fellowship will further his/her work in that field. The Foundation therefore reserves the right to cancel or terminate the Fellowship if the Fellow changes employment or vocation.
4.2The Fellow hereby agrees to be bound by this Contract and Conditions of Award as agreed and signed between the Foundation and the Fellow. It is expected that the Fellow will return to work in the health service for at least one (1) year after the termination of the Fellowship.
4.3Illness which reduces the effective tenure of the Fellowship, shall not automatically entitle the Fellow to an extension of the original period of tenure, but the Foundation may, on application, grant any extension that it thinks fit.
4.4The Fellow shall make adequate arrangements for health and accident insurance prior to taking up the Fellowship, these matters not being the responsibility of the Foundation.
4.5The Fellowship may be suspended or terminated before the approved completion date, either at the Fellow’s request or on the recommendation to the Foundation by the Committee if:
- Performance is unsatisfactory, or
- the Fellow has failed to observe the conditions governing the Fellowship, or
- the Fellow is guilty of misconduct
4.6Within six weeks of the completion of the Fellowship, a written report is to be forwarded to theCYWHS Research Secretariat, Level 2, SamuelWayBuilding, Children, Youth and Women's Health Service, marked Attn: WCH Foundation Trustees–May Fotheringham Fellowship. This report should include a statement that the funds have been expended in line with the conditions of this contract.
4.7Any publication or presentation by the Fellow describing the work supported wholly or partly by the Fellowship shall make due acknowledgment of such support. Copies of any publications are to be submitted to the CYWHS Research Secretariat.
4.8The Fellow shall devote the whole of his/her time to the requirements of the Fellowship. Activities outside the scope of the Fellowship including paid work may not be undertaken except with the written consent of the Foundation.
4.9The Fellow will be responsible for obtaining all necessary passports, visas, etc, required for the country/ countries to be visited. Before departure, the Fellow must submit and obtain the approval of the Chief Executiveor delegate for the final program and any itinerary requiring overseas travel. Any modifications to this final program and itinerary must have the prior approval of the Chief Executive or delegate.
4.10It is essential that Fellowship applicants are endorsed by their Departmental Managers, Divisional Chiefs and relevant Executive Directors and that provision has been made in the Departmental budget process for any leave which may require backfilling during the period of the Fellowship. The WCH Foundation Inc will not provide additional funds to cover backfilling.
