NOTE: If you fill out your application online and return it electronically, please contact us at if you have not heard from us in three days time to ensure your application has been received. Filling out an application does not guarantee adoption of one of our animals, but we do not adopt out any dog without this form completed and a contract signed.
*Please note: We offer a 2 week trial adoption which allows you the opportunity to get a feel for your new dog and to make sure he/she is a good match for your family. If you contact us within 2 weeks of the trial start date and inform us that you wish to return the dog, we will take the dog back at our earliest convenience and return your donation fee. If you do not contact us within the first 2 weeks, we will consider the dog officially adopted and your donation fee becomes non-refundable, even if you return the dog at a later date. If you are doing a trial adoption, the adoption contract applies for the duration of the trial! Under the trial adoption, you are responsible for the animal identified below.

ADOPTION CONTRACT

All Breed Rescue Angles, Inc. (herein afterwards known as “ABRA”), working in conjunction with transport and long distance adoption with different shelters, rescues and organizations, has evaluated, vaccinated, temperament tested, and approved this dog for adoption.
NON-TRANSFERABILITY OF THE ADOPTION:
ABRA policies require all animals adopted from ABRA be adopted to a single individual/family/or other appropriate individual who has been approved by ABRA. I acknowledge my understanding that this adoption is non-transferrable and that any attempt to transfer this adoption to any unauthorized individual may result in a cancellation of this adoption, a forfeiture of the adopted animal, and that any adoption fees paid may not be refunded.
I acknowledge that this dog is being adopted by me as a pet and will not be used in any commercial capacity, including but not limited to: large-scale professional breeding (puppy mills), guard dogs, watch dogs, or in any type of dog fighting or as bait to induce other dogs into fighting.
RETURN OF THE ADOPTED ANIMAL TO ABRA
In the event that I determine that I am unable to keep the animal, I agree that I must first contact ABRA and report any problems/concerns to the foster/adoption coordinator immediately. The foster/coordinator agrees to counsel the new owners concerning their new pet whenever necessary, and to accept animals back into foster care if issues arise following adoption. Animals are accepted back into the program when time, space and/or foster homes are available. ABRA will accept return of adopted animals as space allows, and expressly reserves the right to prioritize shelter animals facing euthanasia above previously adopted animals that are returned. Returning an adopted animal will be considered a last resort and should be pursued only after all attempts at training and consultation with a professional have been exhausted.
In the event that I and ABRA determine that the animal needs to be returned, I also acknowledge that return of the animal is not an immediate process and I may be required to keep, protect, maintain, and care for the animal for up to 10 days, at my own expense, to allow ABRA time to locate a suitable alternative placement for the animal. I further acknowledge my understanding that I am not to give, sell, loan, trade, or transfer the animal to any non-authorized individual, group, organization, corporation, non-profit, government, or commercial entity, without direct approval by ABRA. Failure to comply with these requirements may result in legal action. Animals returned without complete health records and/or with outdated vaccinations and preventative history records will be subjected to a $25.00 owner surrender fee.
LIABILITY
By signing this agreement, I agree not to hold ABRA, other rescues, pounds, animal control facilities, shelters, humane societies, vets, clinics, animal hospitals, transporters, advertisers or foster families associated with this adoption liable for any damages, including loss of personal property, injury to self or others, human or animal.

GENERAL NOTES
It is understood that animal control facilities and transitions into foster homes are very stressful to an animal and underlying medical problems and/or behavioral issues not seen at first may occur after adoption. I agree that I will report any problems/concerns after the adoption to the foster/adoption coordinator immediately. Please note: It may take up to four months, or longer, for a dog to fully adjust to a new home. Please be patient. All available medical records will be provided to the adoptive family at the conclusion of the two week trial period. If your pet requires further medical attention, we will contact you to schedule remainder of services.
