1. Project champion's name and contact information

Name, email, phone number. This is who we'll contact if we have questions. You'll find out NLTOctober 6if your project is selected as a finalist.

2. CIO you're representing

Go you! If your project is selected as a finalist, it doesn't mean that you're the one who has to pitch. You can select someone from your team, or invite your entire team to join you onstage at the competition.

3. Title of your project or idea

Have fun with it. Be colorful.

4. What problem are you trying to solve and why does it matter? (500 words max.)

Describe the challenge you want to address (for whom, what, where). Don't describe the solution. What would happen if your problem were solved? Consider implications for efficiency/effectiveness, cost/time savings, and if it meets the needs of vulnerable populations or large numbers of people.

5. Who is the primary group of a people you are trying to solve a problem for (users)? How do you know this is a problem for them (interviews, surveys, informal feedback)?

Consider speaking with users–this strengthens your application. There may be many groups of people (stakeholders), but focus on the primary group – if they don’t adopt the solution, the entire project fails.

6. What work (if any) is already underway by you/your team? (250 words max.)

Type "none," if applicable. Otherwise, please describe the project's current status. Is it in the planning, development, testing, or implementation phase? Who is working on the project?

7. What challenges might you encounter with this project? What are the potential risks? (250 words max.)

If no, please explain. Our goal is to complete the winning project by August 1, 2018. Please review "If I apply, what is expected of me?” on the CDC Technology Challenge website.
