Congratulations! You have been selected to play for TEAM OKLAHOMA in the 2017 Junior Sunbelt Classic to take place in McAlester and Atoka, OK from Thursday, June 8, 2017 through Wednesday, June 14, 2017.

The Tournament Player Fee is $375. In addition, a $100 deposit for completion of the tournament is required. The total amount of fee and deposit is $475. Upon your team’s last game, the $100 deposit will be returned to you. Please make checks payable to: Sunbelt Classic Baseball Team Oklahoma.
The tournament fee pays for 6 nights with a host family, meals, all local travel Team Oklahoma activities, and Team Gear during the tournament. Parents are responsible for transporting players to and from the Tournament. Please refer to the Schedule of Events for reporting and pickup times.
There will be a Team Oklahoma Workout Day/Practice on Sunday, June 4, 2017, from 12:00 pm to 4:00 pm at Dolese Park in Oklahoma City (uniform pick up and payment due).

1. I am a parent or guardian of the above-named minor who is a participant with TEAM OKLAHOMA.
2. I consent to his participation in all activities included with TEAM OKLAHOMA.
3. I am aware that there are inherent risks and dangers of injury for those involved with TEAM OKLAHOMA. I recognize that such activities are strenuous and may involve intense physical contact.
4. I agree that TEAM OKLAHOMA may rely on my knowledge of the statements listed above and they shall constitute a bar to claims against the coaches for TEAM OKLAHOMA arising out of the player’s participation in suchbaseball activities. I hereby expressly assume the riskof loss resulting from the negligence of the player from the risks incident to participation in TEAM OKLAHOMA baseball activities.

Medical Release Form
I hereby grant permission to the manager and/or coaches of TEAM OKLAHOMA or SUNBELT BASEBALL CLASSIC to obtain medical care from any licensed physician, hospital or medical clinic for me, in the event I am injured or ill and become incapacitated. This authorization is valid only while I am away from my legal address for the purpose of participating in TEAM OKLAHOMA or SUNBELT BASEBALL CLASSIC activities including the period required for travel to and from those activities, and I do hereby waive release, absolve, indemnify and agree to hold harmless TEAM OKLAHOMA or SUNBELT BASEBALL CLASSIC, the organizers, supervisors, participants, and persons transporting me to and from those activities for any claim arising out of an injury to me to the extent, and in the amount covered by accident and/or liability insurance held by TEAM OKLAHOMA or SUNBELT BASEBALL CLASSIC.
