This checklist is not a substitute for official documentation or reading the applicable policies or contracts, but it will help walk you through the process. But don’t hesitate to call for help.
FORMS/DOCUMENTATION THAT ARE USED IN FMLA SITUATIONS
Leave Response/Notification UM 1516 This is the formal documentation to the employee that their time off will be considered FMLA. This can be completed before the person is absent in some cases (planned surgery) but is often sent out after 3 consecutive days absent.
As medical needs change, you should send out revised versions of this form.
Certification of Health Care Provider UM 1515 This is completed by the physician and should be returned to Disability Services. It can be sent from the department however.
NOTE: We will accept an alternate form that a medical provider might use. The form must include specific dates for which the employee is medically unable to work.
Certification for Birth/Care of Newborn UM 1602 This is a short-cut form that can be used in pregnancies without complications. This can be used instead of UM 1515. It can be returned to the department.
Certification of Adoption or Foster Care UM 1603 The short-cut form to use in these circumstances. It can be retuned to the department.

Benefits (at 612-624-9090) – If an employee has elected and paid for Short or Long Term Disability. Hartford Insurance will be the one to actually answer any questions/determine eligibility on those elected plans. Each employee’s health insurance carrier will need to answer any questions related to coverage - supervisors/HR should not be involved with that.
Faculty/P&A Formal Medical Leave is handled within the department.

Employee Assistance (at 6-0253) Employee Assistance is a diagnostic, counseling, and referral service for members of the University's employees who are experiencing personal, relationship, or work related difficulties.
Disability Services – (at 4-3316) Disability Services/Employee Services is the designated office to serve all employees (including faculty and staff) with any disability or temporary medical condition needing accommodations or adjustments. Disability Services obtains and confidentially files disability-related and medical documentation, certifies eligibility for services, assists in determining reasonable accommodations, and develops plans for the provision of such accommodations/adjustments for employees (faculty and staff) with disabilities or medical conditions. They help to determine if disability law is involved. They act as a confidential, neutral party.
Work Comp – if an employee is injured at work which requires him/her to miss more than three (3) days of work, FMLA would run concurrently with any Work Comp.

Office of Environmental Health & Safety – they can assist with work place ergonomics, accommodations, and adaptive technologies that might be needed as a result of a health condition.
