GEM Health Care Educational Nursing Foundation

Helping to Build a Better Future

The GEM Health Care Educational (GHCE) Nursing Foundation was created in 2015 by Gloria and Syed M. Hussain to provide financial means through short term scholarships to two groups of people within GEM Health Care's homes.

1.Internationally trained nurses who are employed at a GEM Health Care facility and who are trying to obtain their RN or LPN/RPN license.

2.Current employees at a GEM Health Care Group facility who are trying to obtain a higher nursing education. For example a CCA/PSW trying to become an LPN/RPN. Or a LPN/RPN who wants to become an RN.

3.Children of a Full Time employee at a GEM Health Care facility who are trying to obtain a college or university education.

Our main objective is to create higher standards of education and professionalism within our GEM facilities. Additionally, we are trying to better the life of our employees by helping them to obtain a better education for themselves, or for their children. We propose to do this by providing financial aid to reduce the burden that impacts families when one is trying to educate themselves or their children.

There will be a Board of Trustees (Board) created that will carry out the duties necessary to screen the applications received and to then select the successful applicants and award allotted funds.

Careers:

The following are a list of careers that are professed by the GHCE Nursing Foundation.

- Bachelor of Nursing (RN)

- Practical Nursing (RPN/LPN)

- Other Health Care profession

Selection Criteria:

The final selection will be made upon a number of criteria that will help the Board in its selection process. The size of the awards will first be determined by the amount of funds allotted for that purpose for a given year, and then finally by the number of successful applicants. The Board will make the final decision on who will qualify for one of the awards, the size of the award, on a case by case basis. The award cannot exceed the amount of the tuition for a term or a year, and in no case shall an individual's award exceed $5,000. The award can be offered to an individual only once. A family may only submit an application for one child per year to be reviewed.

1.If the applicant is currently an employee of a GEM Health Care facility

2.or, if the applicant's parent is a Full Time employee of a GEM Health Care facility

3.The applicant's financial need (proof will be required).

4.That the applicant has a valid Social Insurance Number (SI Number).

8.Any other information that the Board may require to assist it in its deliberations.

How to apply:

In order to apply (make application) to the Board for an award, the applicants must complete the 'Applicant Questionnaire' and provide the necessary supporting documentation. The applicant as part of the Questionnaire will be required to submit a 1,000 to 1,500 word essay. The purpose of the essay is for the applicant to tell the Board about itself. Its hopes and dreams, what it has done to further its education, what affect this education would have on the individual. What other interests or activities the individuals does. The applicant should consider carefully what other information it may want to include as supporting documentation that might assist the Board in making its decision. A fillable PDF version of the Applicant Questionnaire is available from your Administrator.

The applicant should send the completed Applicant Questionnaire to the Board together with all supporting documentation (copies only please, no originals will be returned) and the 1,000 to 1,500 word Essay by courier or registered mail, so that it is received by the Board no later than May 15th 2018 (deadline) for the school year that would start in September 2017.
