GENERAL INSTRUCTIONS
The Sunnyvale Education Foundation (SEF) is committed to supporting the educational goals of Sunnyvale ISD. The TC Lupton Leadership Award is designed to encourage students in their pursuit of higher education. The scholarship is open to all seniors graduating from Sunnyvale ISD each academic year. Criteria have been established to evaluate applicants on both scholastic and leadership achievements. The scholarship will be based on leadership activities, community service, essay, GPA, SAT/ACT scores, and participation in the senior project. The goal of the scholarship program is to promote the next generation of leaders.
A Scholarship Review Committee selected by the SEF Board of Directors will evaluate each application submitted based on the approved criteria. The committee will make a recommendation for scholarship awards to the full SEF Board of Directors for final approval. This will be a one time $2,500 award. Any SEF Director with a child in the graduating class for the academic year in process will not serve on the Scholarship Review Committee and will not cast a vote for final approval.
All awarded scholarships will be a one-time award with the winner announced at the annual awards program. Funds will be paid directly to the winner’s school of choice.
Placement in an alternative school program of any kind disqualifies a student from submitting an application, being considered for an award, receiving an award or receiving a funds disbursement.

Please list the Universities, Colleges, or Schools where you have submitted an application. If you have already been admitted to and selected a program, please provide that information. Also include your potential major or field of study.
LEADERSHIP ACTIVITIES
Use the attached spreadsheets to provide information on the organizations and activities in which you have played a leadership role during your high school years. This can be school, community, religious, work or other organizations. Please be specific on the roles and responsibilities you had with each entry. Include the organization name, role / responsibility, and timeframe for each activity.
COMMUNITY SERVICE INVOLVEMENT
Use the attached spreadsheet to provide a listing of your community service activities during your high school years. Examples include civic and religious organizations, volunteer efforts and employment. Please be specific on the roles and responsibilities you had with each organization. Include the year you participated as well as the number of hours you devoted to this activity.
EDUCATIONAL BACKGROUND
Grade Point Average (GPA)
On your resume include a section listing your current GPA as well as the class rank or class percentile. You also must submit an official Sunnyvale ISD transcript reflecting these scores.
Standardized Test Results
On your resume include a section listing your highest standardized test results. You must also submit documentation supporting those scores. .
SENIOR PROJECT
On your resume include the topic of your senior project, who your mentor was and why you choose them, a brief overview of what you did and a statement on the current status of the project. In order to receive this scholarship your senior project must be completed on time.
ESSAY
On a separate sheet, provide a 500 – 750 word essay. The topic of the essay should revolve around “Making a Difference”. This could be about how you have made a difference, how someone else has made a difference in your life or how you intend to make a difference in the future.
Letter of Recommendation
You must submit a letter of recommendation from a SISD teacher.
Citizenship
You must submit a statement from the SHS counselor listing any serious disciplinary actions you have had in the last 2 years. Serious disciplinary actions include in school and out of school suspensions. These will not disqualify you from receiving the scholarship by may be taken into consideration depending on the circumstances involved.
