Ohio student disclosure course registration fee of $25 as required by rule 3332-1-22.1 of the Administrative Code. To be paid by the student at the time of taking the course online, prior to the 1st day of class. To complete the Disclosure course, go to: and click on Students- enroll in course. Then follow the instructions.

All fees and arrangements must be in place prior to class start.

*Students cannot sit for State Exam if any monies are owed*

Enrollment Requirements:

Must be 16 years or older

Complete Registration Form and Student Enrollment Agreement

Completion of the student disclosure course

Graduation Requirements:

Complete 75 hours of class, skills and labs

Pass total of all tests/exams with 80% or better

Tuition paid in full

School Policies:

Absences– it is prohibited for any student to be absent on either the first or second day of class. The maximum number of days a student may be absent is two (2).

Tardiness– any student who arrives more than 15 minutes after the beginning of the class will be considered tardy.

Make-up Work – Students, who must be absent or otherwise miss class work/tests, must make up said work after class when Instructor is available.

Interruption or termination for unsatisfactory work or attendance – (3 strike rule) student will be given three notifications of unsatisfactory progress/participation. After the third such notification, student will be terminated.

Grading System –90 – 100% = A

80 – 89% = B

70 – 79% = C

Percentages under 80% are unsatisfactory

Unsatisfactory Progress – three exams under 80%, tardiness (three

unexcused), two unexcused absences. Students who reach this

unsatisfactory level of progress will be put on a probationary period

for the duration of class

Conditions of Re-entrance for students dismissed or suspended

for unsatisfactory progress – student must provide documentation

stating the reason/emergency during the applicable time period (prior class).

Academic Progress (for federal and state financial aid programs) – at least 80% on exams, and certificate of completion.

Refund Policy:

The enrollment agreement may be cancelled within five calendar days after the date of signing, provided that the school is notified of the cancellation in writing.

If such cancellation is made, the school will promptly refund in full all tuition and fees paid pursuant to the enrollment agreement and the refund shall be made no later than thirty days after cancellation. This provision shall not apply if the student has already started academic classes.

1. A student who withdraws before the first class and after the 5-daycancellation period shall be obligated for the registration fee.

2. A student who starts class and withdraws before the academic term is 15% completed (12 clock hours) will be obligated for 25% of the tuition and refundable fees plus the registration fee.

3. A student who starts class and withdraws after the academic term is 15% complete but before the academic term is 25% completed (20 clock hours) will be obligated for 50% of the tuition and refundable fees plus the registration fee.

4. A student who starts class and withdraws after the academic term is 25% complete but before the academic term is 40%, completed (32 clock hours) 5 will be obligated for 75% of the tuition and refundable fees plus the registration fee.

5. A student who starts class and withdraws after the academic term is 40% completed will not be entitled to a refund of the tuition and fees.

Grievance Procedure:

All student complaints and grievances should be handled in the following manner:
