READMISSION POLICY

**Student Responsibility - When a student withdraws from the nursing program he/she must also go to the Counselor’s office to withdraw from the college with a grade of "W". If the student fails to do this, he/she will receive a "F" in the course. The student must also complete the “Student Exit Form” found in the handbook if dropping out or stopping out of the nursing program. Students who drop out or stop out and do not complete this form will not be eligible for readmission.

A student may request one readmission to the Associate Degree Nursing Program at Trinity Valley Community College. Students who have been dismissed from the program for unsafe clinical performance or violation of the honesty policy will not be eligible for readmission.

The faculty will evaluate the reason for the student’s withdrawal or failure from the ADN program. Stipulations for return into the program will be delineated. The student may be required to re-enroll in and repeat certain nursing courses. Readmission will be based upon completion of the required stipulations and availability of spaces in the program. In cases of more requests for readmission than the number of spaces available, priority will be given to those with extenuating circumstances, then academic failures, and lastly those students with clinical problems.

To be eligible for readmission, the student must indicate their desire to re-enter in writing to the HSC Provost by the date specified in the stipulation letter. The student may request to meet with an instructor or the Provost for an exit interview. An exit interview is not required.

The student must keep the Health Science Center informed of any changes in e-mail address, address or phone number and must respond in a timely manner to any correspondence sent from TVCC.

Consideration of a second readmission due to extenuating circumstances will be evaluated by written petition to the readmission committee/Provost. Requests for a second readmission will have lower priority than a first readmission.

Students are encouraged to re-enter within 1 year. If they re-enter within 2 years, the student may be required to repeat one or two previous courses, to help ensure their success in the program. They may also be required to pass a pharmacology math test and successfully demonstrate selected skills in order to be able to be re-admitted.

After five years, if a student has had their one re-admission and was not allowed to re-enter, he/she needs to send a letter to the provost requesting re-admission that explains what has changed since the last re-admission. The HSC re-admission committee will consider the request. If the request is approved, the student will need to apply and be considered with the general pool of applicants.

Students may not re-apply to the nursing program using the regular admissions procedure unless it has been five years since their admission. Students should follow the readmission procedure and complete all stipulations.
