3. POSITION DETAILS
Position Context
The services and programs of the ISCHS are currently grouped by target group or work area. The Aged Health & Rehabilitation Program (AH&RP) consists of staff, from various disciplines, located across four Centres. The target groups are primarily the frail aged, people with disabilities and their carers, and include people from culturally and linguistically diverse backgrounds. Services provided include community nursing, social work, physiotherapy, podiatry, dietetics, speech pathology and occupational therapy, with a range of special projects and group activities. The Port Phillip Community Rehabilitation Centre forms part of the Program and is located at the Southport Centre.
This position is funded by the Department of Human Services and aims to assist isolated older people and also assists people with an ABI over 18 years of age to maintain independent, stable housing. The Program works closely with the ISCHS-managed Community Connections Program which provides assertive outreach, referral and support services to people who are homeless or at risk of homelessness. The position is supported within a dynamic and supportive team, and receives strong clinical supervision.
Position Objective
The Case Manager – Older Persons/ABI will provide flexible, comprehensive case management and supportive outreach services to older people and also to younger people with an Acquired Brain Injury (non-compensable). Isolated older people will require engagement and supportive case management, ABI clients will require support services to gain access to, and maintain stable public housing.
The Case Manager, as a member of the AH&RP, will also have a role in the ongoing development of the team and the program and will, at times, work closely with staff members of other programs on client or organisational issues. The worker will contribute to the development of the service system to meet the needs of the target group.
Key Responsibilities
Client Services
· Provide comprehensive and flexible case management services to the target group.
· Provide assertive outreach to support socially isolated clients.
· Provide referral, as appropriate, to services within the ISCHS, as well as to other health and community services.
· Provide direct support services where appropriate, for example, assisting clients to appointments.
· Manage the expenditure of Flexible Care Funds for ABI clients requiring support services
· Participate in various quality assurance processes, including case allocation, case review, and support and supervision as required.
· Maintain case files, documentation and statistics according to ISCHS policy.
Program and Service Development
· Contribute to the ongoing development and evaluation of the Older Persons/ABI Housing Support position
· Develop and maintain strong links with other Aged Health staff and services, the Community Connections Program, the Office of Housing and the Rental Housing Support Program.
· Contribute to the development of relevant inter-agency protocols around service delivery to clients.
· Facilitate the development of joint program initiatives with other service providers and community facilities, as appropriate.
· Contribute to the ongoing development of the AH&RP Program by participating in planning and reviewing services and programs.
· Contribute to ISCHS policy development, working groups and specific task groups as required.
· Participate in, and contribute to, Centre activities which foster the development of the local service profile and service delivery system.
Professional Development
· Attend and contribute to staff development and supervision programs, and to attend relevant courses and mandatory in-house training when appropriate.
· Participate in peer review/practice review.
Occupational Health & Safety
· Ensure that work and services are provided in a safe manner at all times by regularly reviewing practices and environment and by participating in OH&S training where required.
Continuous Quality Improvement
· Identify continuous quality improvement opportunities
· Participate in the development of quality procedures and contribute to internal and external program reviews as required.
Performance Management Framework
· Actively participate in an annual individual performance plan, which determines key responsibilities, strategies and performance indicates in line with the organisation’s Strategic Plan and Program Business Plan.
Health Promotion
· Contribute to the development, implementation and evaluation of organisational Health Promotion strategies as identified in the Health Promotion Plan; and undertake health promotion training as required.
Administrative
· To attend all relevant Centre meetings.
· To accurately complete timesheets and statistics on a routine basis.
General
· To participate in the ongoing planning, evaluation and development of the Aged Health and Rehabilitation Program.
· To adhere to ISCHS policies and procedures.
· To participate in quality assurance activities as required.
· To receive appropriate training about the Needle & Syringe Program (NSP) and participate in the NSP as required.
· To undertake active and meaningful community participation and engagement
· Other duties as required by the Manager of the Program.
3.4 Reporting Relationships
· The Older Persons/ABI Case Manager reports to the Coordinator AH&RP.
4. KEY SELECTION CRITERIA
Mandatory
· Degree in social work or equivalent qualification
· Demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and users.
· An understanding of and commitment to the target group
· Demonstrated knowledge of the aged and disability service system and the public housing sector.
· Demonstrated ability to provide outreach case management services to people with complex needs who are socially isolated and/or have challenging behaviours.
· Demonstrated ability to contribute to the functioning of a multi-disciplinary team.
· Well-developed interpersonal and communication skills.
· Demonstrated understanding of working with people from linguistically and culturally diverse backgrounds.
· Computer literacy skills.
· Current Driver’s licence.
Desirable
· Experience working with and influencing the service system to improve responsiveness to the client group.
· Knowledge of the local service environment (Inner south).
· Demonstrated experience in program planning and evaluation.
· Ability to speak a relevant community language other than English.

Probationary Period / This position is subject to a probationary period of three months, in accordance with ISCHS Recruitment Policy.
Location / The position works across Prahran/South Melbourne/St Kilda/Glen Eira but will be based primarily at Prahran.
From time to time the incumbent may be requested to work from or be based at other ISCHS centres.
Salary Packaging Information:
· Salary packaging is a legitimate method of restructuring existing salary into a combination of salary and expense payment benefits in order to provide a higher net remuneration for the employee.
· This offer applies while Inner South Community Health Service continues to be granted "public benevolent institution" status and enjoys exemption from the provisions of the Fringe Benefits Tax (FBT). Should our status change salary packaging will no longer be available.
· It is not compulsory for employees to take up the offer to ‘package’ their salary; however the benefits from packaging are considerable. There is a small cost to the employee.
· Salary packaging is not available for casual or locum employees.
Meals Entertainment Information:
· Meal entertainment is also available at ISCHS. It is of great benefit to staff as it is a tax free benefit in addition to the $16,050 that you can claim through Salary Packaging. ISCHS staff are able to claim up to $5,000pa worth of FBT exempt meal entertainment expenses on top of their existing packaging.
PLEASE NOTE: There is potential that Community Health may loose its current PBI Status, which will affect Salary Packaging and Meals Entertainment benefits. Community Health Agencies are responding to this potential loss as a sector and are hopeful that we will be able to maintain our Salary Packaging and Meals Entertainment Entitlements.
