VOLUNTEER/OBSERVATION
CHECKLIST
This checklist is to be used for processing onboarding of individuals who will be at Saint Louis University to volunteer, observe or participate in school related internships/externships. These processes do not apply to voluntary faculty and volunteers who have been screened and/or credentialed by another university/school or health care entity.
- For ALL participants the following are needed:
- Human Resources Information Form for Volunteer/Observation Participants
- This form provides important emergency contact information, email, telephone directory, and for University and government reporting.
- The participant should use their full legal name as it appears on their government issued identification.
- The department sponsoring the individual should provide them with start and end dates for their program.
- If the participant has a disability and needs assistance, please have them contact Human Resources
- Volunteer/Observation ParticipantConsent and Liability Release
- This form must be signed by the participant;
- If the participant is under age 18, the form must be signed by a parent or guardian; and
- The sponsoring faculty/staff member must also sign the form.
- A copy of a government issued identification photo card. Acceptable examples include:
- Driver’s license
- Passport
- Visa
- Government ID (non-driver’s license)
- Minors under 18 may provide a copy of their school ID
- Background Checks and Health Screenings
- Participantsmay need to have a Human Resources criminal background check done prior to their participation. This is necessary for those individuals with unsupervised contact with students or patients, and/or those with access to confidential and/or financial information and/or assets.
- They need to set up an appointment with the Office of Clinical Education Compliance by going to the following website:
- Even if the individual is a student, they still need to follow the process for Human Resources background checks to participate as a volunteer, observer, or intern/extern (not the link for student background checks).
- Participants on the SLUCare medical campusshould schedule an appointment for a health screening by calling (314) 268-5499. Prior to participation, the individual must provide documentation showing they have received clearance from Employee Health.
- Confidentiality & Security Agreement - Participants who have access to confidential University and/or protected health information must also sign and submit the Confidentiality & Security Agreement.
