This communication is for all employees who are members of Employees Pension Scheme ‘95 (EPS ’95)

Members who have completed 58 years of age and 10 years of continuous service in the company become eligible to receive the Pension amount which is paid by the Regional Provident Fund Commissioner’s Office. For this purpose members are required to fill up form 10D for monthly pension claim. The guidelines for filling the form are given in the attached file. The pension number will be given by Mrs. R P Hormusji - P & TM Dept.

Members who resign must fill up form 10D for reduced pension if they are 50 years and above and completed more than 10 years continuous service or form 10C for withdrawal/Scheme Certificate.

Members who have completed 58 years of age and 10 years of continuous service and have become eligible to receive the Pension amount are requested to immediately complete the requisite Form and send it to Payroll Section – ATTN Ms R P Hormusji thru the Divisional Accounts and Administration Office. Employees located in Carnac and Bombay House may send the Forms directly to Ms R P Hormusji Payroll & Trust Management – Carnac.

EPS 1995 – CLAIM FORMS

FORM 10D

1)Superannuated employees – On attaining 58 years whether in service or not.

2)Reduced Pension – those who have left the company and not joined any other company eg. Resignation, VRS, ESS, Dismissal and completed 50 years of age and ten years of service, can apply for reduced pension.

3)Widow and children pension – In case of demise of a member, the widow can apply for widow pension and if the children are below 25 years of age the first two children are eligible for pension. On their attaining 25 years of age the others will start getting orphan pension till they attain 25 years of age.

4)Orphan Pension – In case of demise of a member, if the spouse has also died the orphan gets the pension if he/she is less than 25 years of age. In case of minor the guardian will have to obtain heirship certificate from the court.

5)Disablement Pension – If the member has taken VRMG and is above 50 years and completed 10 years of service, he does not need ESIS Doctor’s certificate. But if the member is less than 50 years, he has to obtain ESIS Doctor’s certificate from J J Hospital and get disablement pension.

Instructions for filling the form 10D :

Item 1 – By whom the pension is claimed? - “Member” in case of all except demise case “Widow” or “Widow & children”.

Item 2 - Type of Pension claimed - Any one of the abovementioned Five types of Pensions.

Item 3 (a) to (e) – all particulars of the member should be filled. In marital status, if legally separated please do not write married.

Item 4 EPF Account Number - Please leave blank if not available, PF dept will do the needful or check with the PF dept and then write.

Item 5 - Put the stamp of the respective division in which the member was last working.

Item 6 – For superannuated member eg. If a member completes 58 years of age on 15/11/05 please write 30/11/2005. Do not write the date of retirement as that will be two years after attaining the age of 58.

If a member dies after retirement and if the widow is filling the form, the date of separation should be superannuated date and not date of demise.

Item 7 - Appropriate reason of leaving should be written.

Item 8 – Address of the member should be written.

Item 9 -OPTION FOR COMMUTATION OF 1/3RD OF PENSION (CAN OPT FOR LESSER COMMUTATION ALSO)

If the member is eligible for Rs.1200 pension per month, then he will get 1200/3*100=Rs.40,000/- in the beginning and Rs.1200-400=Rs.800/- as pension throughout his life.

ITEM 10 -option for return of capital- Only a member can opt for ROC, a widow cannot opt.

option 1 :

The member can opt for 10% less from the monthly pension i.e. if the pension is Rs.1200, less Rs120= Rs1080 per month. On death of the member the nominee (anyone) will receive 100 times of monthly pension i.e. Rs.1200*100=Rs.1,20,000/- PLUS widow pension (50% of 1200) PLUS child pension (25% of 1200).

option 2

Same as option 1, but on death of member widow will get (80% of 1200=Rs960) PLUS child pension (50% of 1200=600) PLUS ROC 90 times of pension ie. 90*1200=1,08,000 to nominee (anyone except wife)

option 3

Monthly member’s pension paid for 20 years certain @ 87.5% i.e. 87.5% of 1200 . If he dies before 20 years, his wife will continue to get that pension amount. After 20 years the nominee (anyone) will get ROC i.e. 87.5*1200=Rs.1,05,000

Item 11 - Only if a member has opted for ROC he has to nominate wife in case of Opt 1 & 3, for Opt 2, he cannot nominate his wife.

Item 12 –If a member has four children below 25 years of age, all four children’s name should be mentioned in item 12 wirh their date of birth and age proof like birth certificate or school leaving certificate.

Item 13 – if a member has retired and then expired the date of death should be mentioned here.

Item 14 - details of saving bank account number :

The saving bank account should be purely for Provident fund pension and should be in single name in the prescribed banks only i.e. Bank of India, Punjab National Bank, HDFC, State Bank of India, ICICI, purely for pension in single name.

If case of death of the member, the pensioner/s, all the claimants should open the account in the same bank with different account numbers. In case of minor child , the child’s name first and guardian’s name second.

Descriptive Role Form – If the member is claiming the pension, one form in duplicate should be filled and signed by the respective division’s authorized signatory.

If the pension is claimed by widow as well as children, then the descriptive role form should be filled by widow separately and each child separately. In case of minor the mother/gaurdian can sign the descriptive role form.

1)Photographs

In case of member claiming the pension 3 photographs passport size of member and spouse together. At the back of the photgraph, his pension number and name should be written and attested by the signatory. In case of widow and children pension (joint photo of widow with minor child and child above 18 years of age separate photographs).

Proof of age of all children claiming pension.

Original Death Certificate in case of member’s demise.

If there are more than one claimants, the descriptive role form should be Xeroxed and filled by each claimant separately in duplicate

FORM 10 C

This form has to be filled by members on separation, if he/she is less than 50 years of age and not completed 10 years service.

This form has to be filled either for obtaining Scheme Certificate or Withdrawal.

Scheme Certificate - If a member resigns and joins any company in India, should obtain SC, so that when he/she attains 58 years age, can apply for pension claim where service in Tata Power is also considered.

Widhrawal - When a member resigns without completing 10 yrs service and is settling abroad, he/she can withdraw the pension amount.
